[Objective] To explore the pathology and the injury mechanism, also the operative result of long-segment cervical spinal cord injury without fracture or dislocation. [Methods] Thirty one cases of long-segment cervical spinal cord injury without fracture or dislocation included 25cases(80.65%)with congenital and secondary cervical spinal stenosis, 3cases(9.68%)complicated with posterior longitudinal ligament calcification and 2cases(6.45%)complicated with multiple cervical disc protrusion. All patients were operated by posterior wide decompression and 1atera1 mass screw fixation. The changes of spinal function after operation were observed and the efficacy was assessed. [Results] All patients were followed up for 3~l7 months(average 9months). The average preoperative ASIA score was 96.63±6.4 points. After 3 weeks and 3 months, the average ASIA score was 158.53± 12.4 points and 181.68±12.6 points respectively. There were significant differences(P<0.05) in ASIA score between preoperation and after 3 weeks and 3 months. There was no significant difference (P>0.05)in ASIA score between after 3 weeks and 3 months.Except 2 cases without improvement, the others had different degrees of restoration. [Conc1usion] Cervica1 canal stenosis is the main reason of the cervical spinal cord injury without fracture or dislocation. The acting factors are the inertia and the direct outside force. Operation with posterior wide decompression and 1ateral mass screw fixation can achieve good recovery of spinal cord function.
