目的:评估脂肪阿拉伯酸甘露聚糖抗体(LAM-IgG)检测对结核性脑膜炎的诊断价值。方法:酶联免疫吸附试验定性检测血清和脑脊液中LAM-IgG0结果:77例结核性脑膜炎病例中,血清LAM-IgG阳性率为45.45%,30例对照组阳性率为3.33%,特异性为96.67%;脑脊液LAM-IgG阳性率为48.05%,特异性为l00%,两者间差异无显著性(x2=0.0714,0.90>P>0.75)。同时检测血清和脑脊液LAM-IgG,阳性率为55.84%,特异性为96.67%,与单独检测脑脊液LAM-IgG结果比较,两者差异有显著性(χ2=4.1667,P<0.05)。结论:检测血清或脑脊液中LAM-IgG对结核性脑膜炎的诊断有重要临床价值,同时检测血清和脑脊液中LAM-IgG可提高结核性脑膜炎的诊断率。

목적:평고지방아랍백산감로취당항체(LAM-IgG)검측대결핵성뇌막염적진단개치。방법:매련면역흡부시험정성검측혈청화뇌척액중LAM-IgG0결과:77례결핵성뇌막염병례중,혈청LAM-IgG양성솔위45.45%,30례대조조양성솔위3.33%,특이성위96.67%;뇌척액LAM-IgG양성솔위48.05%,특이성위l00%,량자간차이무현저성(x2=0.0714,0.90>P>0.75)。동시검측혈청화뇌척액LAM-IgG,양성솔위55.84%,특이성위96.67%,여단독검측뇌척액LAM-IgG결과비교,량자차이유현저성(χ2=4.1667,P<0.05)。결론:검측혈청혹뇌척액중LAM-IgG대결핵성뇌막염적진단유중요림상개치,동시검측혈청화뇌척액중LAM-IgG가제고결핵성뇌막염적진단솔。
Objective:To ascertain the value of lipoarabinomannan-IgG(LAM-IgG) determination in the diagnosis of tuberculous meningitis. Methods:LAM-IgG in serum and cerebrospinal fluid was detected with enzyme-linked immunosorbent assay. Results: In 77 patients with tuberculous meningitis,The positive rate of LAM-IgG in serum was 45.459% ,the positive rate in 30 control cases was 3.33% ,the specificity was 96.67% ;the positive rate of LAM-IgG in cerebrospinal fluid was 48.05% ,the specificitywas 100 %, their differences were not remarkable (χ2 = 0. 0714 , 0. 90 > P > 0.75). Detecting LAM-IgG in serum and cerebrospinalfluid at same time, the positive rate was 55. 84 %, the specificity was 96.67 %, comparing with the positive rate of LAM-IgG in cerebrospinal fluid,their difference was remarkable (χ2= 4.1667,P< 0.05). Conclusions: Detection of LAM-IgG in serum or cerbrospinal fluid have importance clinic value for diagnosing tuberculous meningitis,and detecting LAM-IgG in serum and cerebrospinalfluid at same time can increase diagnostic rate of tuberculous meningitis.
