目的 通过建立犬脑顽固性颅高压模型,从基础实验层面出发,加深了解外伤后顽固性颅高压的病理生理学机制.方法 杂种犬10只,制作脑硬膜外水囊压迫模型,动态监测压迫及解压后3h过程中的动脉压、颅内压、脑电图等多种神经生理指标变化情况.结果 (1)在水囊体积最大时刻,平均动脉压( 97.5±7.8) mmHg及脑温(35.6±0.36)℃分别较其基础值(83.7±13.0) mmHg、(36.8±0.30)℃有显著升高(P=0.023)及降低(P<0.05).(2)随着水囊体积增大,颅内压逐渐升高伴脑灌注压不断降低.(3)解压后,再次出现类似水囊压迫的二次顽固性颅高压(62.2±6.6)mmHg.结论 本实验初步复制出了类似于人类脑外伤后继发性顽固性颅内高压的动物模型,具有较好的可靠性及可重复性.多项生理指标的监测为进一步加深了解该发病机制提供了可靠的依据.
目的 通過建立犬腦頑固性顱高壓模型,從基礎實驗層麵齣髮,加深瞭解外傷後頑固性顱高壓的病理生理學機製.方法 雜種犬10隻,製作腦硬膜外水囊壓迫模型,動態鑑測壓迫及解壓後3h過程中的動脈壓、顱內壓、腦電圖等多種神經生理指標變化情況.結果 (1)在水囊體積最大時刻,平均動脈壓( 97.5±7.8) mmHg及腦溫(35.6±0.36)℃分彆較其基礎值(83.7±13.0) mmHg、(36.8±0.30)℃有顯著升高(P=0.023)及降低(P<0.05).(2)隨著水囊體積增大,顱內壓逐漸升高伴腦灌註壓不斷降低.(3)解壓後,再次齣現類似水囊壓迫的二次頑固性顱高壓(62.2±6.6)mmHg.結論 本實驗初步複製齣瞭類似于人類腦外傷後繼髮性頑固性顱內高壓的動物模型,具有較好的可靠性及可重複性.多項生理指標的鑑測為進一步加深瞭解該髮病機製提供瞭可靠的依據.
목적 통과건립견뇌완고성로고압모형,종기출실험층면출발,가심료해외상후완고성로고압적병리생이학궤제.방법 잡충견10지,제작뇌경막외수낭압박모형,동태감측압박급해압후3h과정중적동맥압、로내압、뇌전도등다충신경생리지표변화정황.결과 (1)재수낭체적최대시각,평균동맥압( 97.5±7.8) mmHg급뇌온(35.6±0.36)℃분별교기기출치(83.7±13.0) mmHg、(36.8±0.30)℃유현저승고(P=0.023)급강저(P<0.05).(2)수착수낭체적증대,로내압축점승고반뇌관주압불단강저.(3)해압후,재차출현유사수낭압박적이차완고성로고압(62.2±6.6)mmHg.결론 본실험초보복제출료유사우인류뇌외상후계발성완고성로내고압적동물모형,구유교호적가고성급가중복성.다항생리지표적감측위진일보가심료해해발병궤제제공료가고적의거.
Objective We expect to have a profound understanding of refractory high intracranial pressure after traumatic brain injury by making a reproducible animal model in dogs experimentally.Method Ten adult dogs were used to make acute epidural balloon compression and decompression models.Changes in mean arterial blood pressure,intracranial pressure and electroencephalography activity were evaluated during the whole compression and three hours after decompression course.Results ( 1 )The mean arterial blood pressure (97.5 ± 7.8 ) mmHg and brain temperature ( 35.6 ± 0.36 ) ℃ had significant increase( P =0.023) and decrease( P < 0.05) respectively in the moment of maximum balloon volume comparing their baseline level ( 83.7 ± 13.0 ) mmHg,( 36.8 ± 0.30 ) ℃.( 2 ) With increase of the balloon volume,ICP rose gradually with cerebral perfusion pressure inversely changed.( 3 ) During decompression,a secondary refractory high intracranial pressure similar to that of balloon compression period occurred (62.2 ± 6.6 ) mmHg.Conclusions Our experiment basically produced a dog model of refractory high intracranial pressure after traumatic brain injury which is similar to the human head injury.It has good reliability and reproducibility.Monitoring of various physiological parameters can provide reasonable evidences for a deeper understanding of its pathogenesis.
