目的 探讨单核细胞趋化蛋白-1(MCP-1)在溃疡性结肠炎(UC)发病机制中的作用.方法 32例UC黏膜标本取自行结肠镜检查的患者,按病理组织学对炎症进行分级,Ⅲ、Ⅳ级20 例,Ⅰ、Ⅱ级 12例;对照组为15名健康成人.应用半定量RT-PCR检测对照组和UC组肠黏膜MCP-1 mRNA表达水平,免疫组织化学方法检测MCP-1蛋白的表达.结果 与对照组相比,UC肠黏膜MCP-1 mRNA和其蛋白过度表达,MCP-1 mRNA表达与疾病严重程度成正相关(P<0.01).MCP-1蛋白表达位于UC肠黏膜固有层单核细胞和T淋巴细胞,病理分级Ⅲ、Ⅳ级肠黏膜MCP-1蛋白表达较病理分级Ⅰ、Ⅱ级组明显增加(t=7.31,P<0.01).结论 UC黏膜组织中MCP-1表达水平明显上调,其表达水平与病情轻重和炎症严重程度呈正相关.

Objective To investigate the expression of monocyte chemoattractant protein 1(MCP-1)in the colonic mucosa of patients with ulcerative colitis(uc),and the correlation with the degree of inflammation in the co-Ionic mucosa and clinical severity of UC and the effect of MCP-1 in the pathogenesis of UC.Methods Colonic muco-sa were obtained from 32 patients with UC who underwent colonoscopy.Patients were also classified histologically as inflammation grade Ⅲ、Ⅳ(20 cases)and inflammation grade Ⅰ,Ⅱ(12 cases).Normal colonic tissues were obtained from 15 healthy adults.Semi-quantitative Reverse transcription polymerase chain reaction(RT-PCR)and immunohis-tochemistry were used to determine the expression of MCP-1.Results Both MCP-1 mRNA and protein expression lev-el were significantly enhanced in mucosa of UC compared with those of healthy control.The level of MCP-1 mRNA ex-pression Was positively correlated to the severity of UC(P<0.01).Immunohistochemisty results showed moderate to strong MCP-1 immunoreactivity which Wag localized in mononuclear cells and T lymphocytes of lamina propria in UC.Patients with inflammation grade Ⅲ,Ⅳ exhibited significantly higher expression of MCP-1 than those with inflamma-tion grade Ⅰ,Ⅱ(t=7.31,P<0.01).Conclusions MCP-1 is overexpressed in colonic mucosa in patients with UC,and the intensity of MCP-1 expression is positively correlated with the degrees of inflammation and the severity of disease.
