目的:探讨儿童以癫痫为首发症状的微小型脑肉芽肿的显微手术治疗方法.方法:根据颅脑CT扫描所见,采用显微操作技术完整切除病灶.结果:9例均在显微镜下操作将病灶完整切除,7例随访8个月～9年,并经CT复查证实无复发,癫痫消失.结论:对儿童癫痫,尤其是来自疫区农村,均应行颅脑CT扫描检查,必要时强化.对此类小占位病灶采用显微手术下切除病灶具有创伤小,保护脑重要功能区及减轻脑水肿等优点.

Objective:To introduce the microsurgical technique to remove the tiny cerebral granuloma with epilepsy as the first symptom.Methods:The lesion was completely removed by microsurgery according to CT scan in 9 children.Results:All children had uneventful postoperative course.The follow-up results of 7 cases with a period from 8 months to 9 years showed disappearance of epilepsy and no relapse of granuloma.Conclusion,CT scan or intensive CT scan should be done for children with epilepsy.The advantages of resection of tiny cerebral granuloma by microsurgery technique are protection from cerebral functional damage,less operative trauma and cerebral edema.
