目的 探讨新疆反复呼吸道感染儿童(复感儿)血清甘露聚糖结合凝集素(MBL)水平与MBL第一外显子54密码子基因多态性的关系.方法 用酶联免疫分析方法检测6个月至10岁复感儿73例(复感儿组)与同龄健康儿童92例(对照组)的血清MBL水平;用聚合酶链反应(PCR) -限制性内切酶片段长度多态性分析(RFLP)方法检测上述复感儿中73例的MBL第一外显子54密码子基因多态性以及突变频率.结果 复感儿血清MBL水平为(2.2±1.2)ng/L,健康儿童血清MBL水平为(3.3±2.1)ng/L,2组差异有统计学意义(P<0.05).复感儿血清MBL水平明显低于健康儿童.新疆复感儿MBL-54密码子突变频率为27.4%,结果与国内报道的健康儿童突变频率1.8%相比较,明显高于国内正常儿童(P<0.05).复感儿MBL-54密码子突变的频率在维吾尔族(9%,7/22)和汉族(18%,13/51)间的差异无统计学意义;不同性别的复感儿MBL-54密码子突变的频率差异无统计学意义.结论 新疆复感儿MBL-54密码子突变频率高于健康儿童,使得复感儿血清MBL水平较正常儿童低,造成免疫水平低下,是这些儿童表现反复呼吸道感染原因之一.

Objective To investigate the relations between gene polymorphism at code 54 of exon 1 and serum protein level of mannose binding lectin(MBL) in children with recurrent respiratory tract infections (RRTI).Methods To detect the serum MBL level and observe the mutation on code 54 of exon 1 of MBL in 92 healthy children and 77 RRTI with enzyme-linked immunoabsordent assay and polymerasechain reaction( PCR)-restriction fragment length polymorphism (RFLP).Results The evaluation in healthy children and children with RRTI was (3.3 ±2.1)ng/L and (2.2 ± 1.2)ng/L,respectively.There was a significant difference (P <0.05).Serum MBL level in children with RRT were lower than that in healthy children.Frequencies at code 54 exon 1 of children with RRTI in xinjiang were 27.4%,which was higher in healthy children of home which was 1.8%.Frequencies at code 54 exon 1 did not show significant difference between Uyghur and Han nationalities.The mutation frequencies at the code 54 of exon 1 did not show significant difference between male and female.Conclusion The mutation on code 54 of exon 1 of MBL is the important cause of the low serum MBL level in children with RRTI.
