目的探讨鼻内镜手术治疗慢性鼻窦炎和鼻息肉的疗效及影响手术疗效的相关因素.方法对2002年-2010年完成随访的527例行鼻内镜鼻窦手术的患者进行临床分析.527例中,Ⅰ型179例,Ⅱ型289例,Ⅲ型59例.498例采用局部麻醉辅以静脉强化麻醉下完成手术.术后随访6～36个月,平均13个月.结果 Ⅰ型治愈率为78.2％,Ⅱ型治愈率为67.1％,Ⅲ型治愈率为55.9％.三型之间治愈率差异有极显著性(P＜ 0.01).手术并发症的发生率为5.9％.结论鼻内镜手术是治疗慢性鼻窦炎和鼻息肉的有效方法,手术疗效与其临床分型及手术处理方式密切相关.

Objective To investigate the curative effect of endoscopic sinus surgery in the treatment of chronic sinusitis and nasal polyps and to explore the related actors affecting surgical efficacy.Methods The data on 527 patients who had undergone endoscopic sinus surgery during the period of 2002 to 2010 were analyzed.Of 527 patients,179 were type Ⅰ,289 were type Ⅱ,and 59 were type Ⅲ.498 patients underwent local anesthesia and intravenous anesthesia.All the patients were followed up for 6 to 36 months,with an average of 13 months.Results The cure rate was 78.2％ for type Ⅰ,67.1％ for type Ⅱ,and 55.9％ for type Ⅲ; and there were significant difference in the cure rate among the three types ( P ＜ 0.01 ).The rate of surgical complication was 5.9％.Conclusions Endoscopic sinus surgery is an effective approach in the treatment of chronic sinusitis and nasal polyps.The surgical efficacy is closely related to the chnical types and the surgical procedures.
