目的 探讨婴幼儿食用受三聚氰胺污染奶粉致急性肾后性尿闭的治疗.方法 对2008年1月1日至2008年9月30日诊治的17例存在上尿路结石梗阻婴幼儿病例进行回顾性分析.结果 13例保守治疗,4例行开放手术取石、解除梗阻.所有病例解除梗阻后肾功能明显改善,血钾恢复正常;随访1~9个月,尿流及肾功能均恢复正常.结论 婴幼儿食用含三聚氰胺奶粉所致结石,部分可通过水化、碱化等保守治疗,部分需急诊手术取石解除梗阻、挽救肾功能,且术中可观察是否合并上尿路畸形并予对症处理,预后良好.
目的 探討嬰幼兒食用受三聚氰胺汙染奶粉緻急性腎後性尿閉的治療.方法 對2008年1月1日至2008年9月30日診治的17例存在上尿路結石梗阻嬰幼兒病例進行迴顧性分析.結果 13例保守治療,4例行開放手術取石、解除梗阻.所有病例解除梗阻後腎功能明顯改善,血鉀恢複正常;隨訪1~9箇月,尿流及腎功能均恢複正常.結論 嬰幼兒食用含三聚氰胺奶粉所緻結石,部分可通過水化、堿化等保守治療,部分需急診手術取石解除梗阻、輓救腎功能,且術中可觀察是否閤併上尿路畸形併予對癥處理,預後良好.

Objective To explore a reasonable treatment for children with acute renal failure due to formula powdered milk contaminated by melamine. Methods From May 1st to September 30th of 2008,17 infants with acute anuria or renal failure who were fed with powdered milk contaminated by melamine were admitted to this hospital. The clinical manifestations, imageologic findings and emergent management of these cases were retrospectively analyzed. Results Obstruction of upper urinary tract was found in all patients. All patients suffered impairment of renal function and blood kalium abnor- mality. Of the seventeen cases, 13 were treated with conservative methods, the other 4 cases were trea- ted with operations. The renal function and blood kalium level of all patients recovered back to normal at 24 hours after treatment. All patients were followed up for 1 to 9 months, and were found to gain normal urinary flow and renal function. Conclusions Of the infants who suffered acute post renal fail- ure due to formula milk contaminated by melamine, some can get recovery by conservative treatments such as hydration and urine kaline, while others need emergent operations to relieve obstruction.
