目的 了解不动杆菌导致医院获得性肺炎的临床特点及其对临床常用抗生素的耐药性。方法 采用Kirby-Bauer琼脂扩散法,按NCCLS1993年标准判断结果,检测医院获得性肺炎患者痰中不动杆菌33株。结果 33例患者发病前均有使用一种或多种抗生素病史,84.85%为COPD,54.55%有混合感染,死亡率45.45%。33株不动杆菌对氨苄西林、哌啦西林耐药率较高(75.76%,54.55%),对庆大霉素、头孢唑啉、阿米卡星、头孢哌酮、头孢三嗪耐药率为18.18%~27.27%,对氧氟沙星、环丙沙星、头孢噻肟耐药效率低于10%。结论 不动杆菌在医院获得性肺炎中占有重要位置,病情复杂,死亡率高。不动杆菌对多种抗生素有不同程度耐药,对氧氟沙星、环丙沙星及三代头孢菌素敏感性较强。

Objective To find the resistance to commonly used antibiotics andthe clinical speciality of nosocomial pheumonia caused by acinetobacer.Methods 33 clinical isolates were collected from sputum of patients with nosocomial pheumonia.Bacterial susceptibility testing was carried out using Kirby-Bauer method.Eleven different antibiotic discs were used.Results At least one kind of antibiotic were used in all cases.28 patients were COPD,making up 84.85%.54.55%of the patients had mixed infection,the mortality was 45.45%.Ofloxacin,ciprofloxacin,cefotaxime showed a very good antibacterial activity against acinetobacter,Ampicillin and piperacillin showed a quite low activity.Conclusion Acinetobacter played an important role in nosocomial pneumonia.Ofloxacin,ciprofloxacin and the third generation cepalosporins can be used for empirical treatment of the nosocomial peumonia caused by acinetobacter.
