目的研究结节性甲状腺肿患者血清胰岛素样生长因子-1(IGF-1)水平与甲状腺摄131 Ⅰ率的相关性,为临床鉴别甲状腺结节的性质寻找更简捷、安全的方法.方法 60例甲状腺结节患者分为热结节组(30例)和冷结节组(30例),以常规体检的健康人员30例作为对照组.采用放射免疫分析法检测血清IGF-1、游离三碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)、TSH的含量,常规放射性核素扫描技术检测甲状腺摄131 Ⅰ率.结果与对照组比较,热结节组患者血清IGF-1 [(315.86±22.74)μg/L]、FT3[(15.32±8.65)pmol/L]、FT4[(86.82±35.22)pmol/L]水平明显增高,摄131 Ⅰ率(0.64±0.17)明显增高,血清TSH[(0.28±0.31)mU/L]水平明显降低,P<0.01.热结节组IGF-1水平与摄131 Ⅰ率呈正相关(r=0.835),与TSH水平呈轻度负相关(r=-0.326).而冷结节组患者血清IGF-1、FT3、FT4、TSH水平与对照组比较差异无统计学意义(P>0.05).结论血清IGF-1水平与甲状腺摄131 Ⅰ率有一定的相关性.
목적 연구결절성갑상선종환자혈청이도소양생장인자-1(IGF-1)수평여갑상선섭131 Ⅰ솔적상관성,위림상감별갑상선결절적성질심조경간첩、안전적방법.방법 60례갑상선결절환자분위열결절조(30례)화랭결절조(30례),이상규체검적건강인원30례작위대조조.채용방사면역분석법검측혈청IGF-1、유리삼전갑상선원안산(FT3)、유리갑상선소(FT4)、TSH적함량,상규방사성핵소소묘기술검측갑상선섭131 Ⅰ솔.결과 여대조조비교,열결절조환자혈청IGF-1 [(315.86±22.74)μg/L]、FT3[(15.32±8.65)pmol/L]、FT4[(86.82±35.22)pmol/L]수평명현증고,섭131 Ⅰ솔(0.64±0.17)명현증고,혈청TSH[(0.28±0.31)mU/L]수평명현강저,P<0.01.열결절조IGF-1수평여섭131 Ⅰ솔정정상관(r=0.835),여TSH수평정경도부상관(r=-0.326).이랭결절조환자혈청IGF-1、FT3、FT4、TSH수평여대조조비교차이무통계학의의(P>0.05).결론 혈청IGF-1수평여갑상선섭131 Ⅰ솔유일정적상관성.
Objective To study the correlation of the serum insulin-like growth factor-1(IGF-1) level and the radioactive iodine uptake rate in patients with nodular goiter,to explore a simple and safty methods of identifying different kinds of thyroid nodule.Methods Sixty diagnosed cases was selected into hot nodule group(30 cases)and cool nodule group(30 cases),and 30 healthy people were selected into the healthy control group.Measured the IGF-1,FT3,FT4,TSH level of each group by radioimmunoassay,and measured the radioactive iodine uptake rate by radionuclear technique.Results To compare with healthy control group,the levels of serum IGF-1,FT3,FT4 and the radioactive iodine uptake rate of hot nodule group were increased obviously,but the serum TSH was decreased significantly,and the level of IGF-1 had positive correhtion with the radioactive iodine uptake rate(r=0.835)in the same group.Meanwhile,the 1 evel of IGF-1 had negative correlation with the level of TSH(r=-0.326).In cool nodule group,the levels of serum IGF-1,FT3,FT4,TSH had no significant difference oompared with healthy control group(P>0.05).Conclusion The level of IGF-1 and radioactive iodine uptake rate in patients with nodular goiter has certain correlation.
