目的 探讨不同节段脊柱结核治疗的手术策略及内固定适应证的选择.方法 回顾分析2001年1月至2005年12月期间接受病灶清除,植骨融合及内固定手术治疗并获得随访的33例脊柱结核患者的临床资料,其中22例颈椎、胸椎和胸腰椎及8例腰椎结核患者接受前路一期病灶清除、植骨融合、内同定术,11例下腰椎和腰骶椎结核患者接受后路椎弓根固定后外侧植骨融合联合前路病灶清除椎间植骨融合术.观察术后和随访期间结核中毒症状和神经功能的改善情况、植骨融合情况及治愈率.结果 术后结核中毒症状明显缓解,神经功能明显改善,25个月时97.0%(32/33)植骨融合,治愈率97.0%.结论 脊柱结核手术治疗中,内固定的应用是安全有效的,手术策略的制定和内固定选择应根据病变节段、脓肿大小、范围等综合判断.

Objective To discusses the different stage tuberculosis treatment the surgery strategy and in the fixed adaptation card choice.Methods The review analyzed from January,2001 to Decembcr,2005 the pertod accepted the focus of infeotion to eliminate,plants 33 cases tuberculosis patient's clnical material which the bone fusion and in the fixed surgery treated and obtains the revisit,22 cases cervical vertebra,the thoracic vertebra and the chest 1 umbar vertebra and 8 cases lumbar vertebra tuberculosis patient accepted a road ahead issue of focus of infeetion to ellnllnate,to plant the bone fusion,in the pexia,after under 11 cases the lumbar vertebra and the 1 umbosacral tuberculosis patient accepted the rear guard vertebral arch root fixed flank to plant the bone fusion union road ahead focus of infection elimination intervertebral to plant the bone fusion technique.After the observation technique and the revisit period tuberculosis poison symptom and the nerve function improvement situation,plants the bone fusion situation and the cure rate.Results After the technique the tuberculosis poison aymptom obvious all eviation,the nerve function distinct improvement,25 months boar 97.0%(32/33) plant the bone fusion,the cure rate 97.0%.Conclusion The tuberculosis surgery treats,in the fixed application is safe effective,the surgery strategy formulation and in the fixed choice should act acording to the pathological change stage,abscess size,the scope and so on the synthetic judgment.
