目的评价微创经皮肾镜碎石术治疗复杂性肾结石的疗效.方法回顾性分析2006年10月至2008年10月采用微创经皮肾镜下碎石术治疗的复杂性肾结石患者68例临床资料.其中单用超声碎石36例,气压弹道联合超声碎石32例.结果 68例患者均成功建立皮肾通道,手术时间30～140 min,平均(80±10)min,结石处理时间平均为(58±9)min.单用超声碎石患者15例术后有部分结石残留,占22.06%;联合气压弹道碎石患者5例术后有部分结石残留,占7.35%.经原通道二期碎石或行体外震波碎石术,均获治愈.随诊1～10个月,无严重手术并发症发生.结论微创经皮肾镜碎石术处理复杂性肾结石,具有创伤小,恢复快,并发症少,较安全,清除结石效率高,可重复的优点.

Objective To evaluate the clinical efficacy of lithotripsy under percutaneousne phrolithotomy (PCNL). Methods From October 2006 to October 2008, 68 cases with complicated nephrolith underwent under PCNL, induding 36 cases of ultrasoniclitho and 32 cases of combined pneumatic and ultrasoniclitho tripsy. Results All thepercutaneous renal tunnel was successfully established, of which residuals tones was generated in 15 cases with ultrasoniclitho account 22.06% and residuals tones was generated in 5 cases with combined pneumatic and ultrasoniclitho tripsy account 7.35% simultaneously, Cure needed second lithotripsy or ESWL later. Operation time ranged from 30 to 140 minutes, with (80 ± 10)minutes averagely. The lithotripsy time was (58 ± 9)minutes. No complication occurred during 1 to 10 months follow-up. Conclusion The lithotripsy under percutaneous nephrolithomy appears valid and provides fast recovery, high cleaning rate, few complication and safety for treatment of complicated renal nephrolith.
