目的 探讨超敏C反应蛋白(hs-CRP)、肌钙蛋白I(cTnI) 心肌酶在新生儿缺血缺氧性脑病(HIE)中的变化及其意义.方法 利用全自动生化分析仪测定35例HIE和26例正常新生儿血清hs-CRP、cTnI水平,并同时检测血清CK、CK-MB、LDH、ALT、AST.结果 HIE新生儿急性期血清hs-CRP水平明显高于对照组(P<0.05);恢复期下降,与正常对照组差异无统计学意义(P>0.05).HIE新生儿急性期血清cTnI水平明显高于对照组(P<0.01);恢复期cTnI与正常对照组之间无差异无统计学意义(P>0.05).HIE新生儿急性期CK、CK-MB、LDH、AST、ALT均较正常新生儿升高;恢复期CK、CK-MB、ALT降低明显(P<0.05),与正常对照组之间差异有统计学意义(P<0.05);LDH、AST降低不明显(P>0.05),但与正常对照组比较差异仍有统计学意义(P<0.05).结论 hs-CRP和cTnI的动态变化与HIE的疗程相平行,能较敏感地反映HIE患儿病情程度及转归趋势,与血清酶学改变一起可作为了解HIE的疗效及病情监测的指标之一.

Objective To investigate the change and clinical significance of serum high-sensitivity C reactive protein (hs-CRP) and cardiac troponin (cTnⅠ) in newborns with hypoxic ischemic encephalopathy.Methods The serum hs-CRP,cTn, creatine kinase (CK), creatine phosphokinase iscenzyine (CK-MB), lactate dehydrogenase (LDH), alanine transaminase (ALT) and aspartate aminctransferasc (AST) were examined by reaction rate method.Results The hs-CRP in newborns with acute HIE was significantly higher than that in control group (P<0.05), and there was no significant difference in recovery stage between two groups. The cTnI in newborns with acute HIE was significantly higher than that in control group (P<0.05), and there was no significant difference in recovery stage between two groups. The CK、CK-MB、LDH、AST and ALT in newborns with acute HIE was significantly higher than that in control group. In recovery stage CK、CK-MB、ALT decreased significantly (P<0.05). LDH and AST decreased insignificantly but with significant difference between two groups. Conclusions The changes of hs-CRP and cTnI in serum can reflect the disease severity and prognosis, and they are importannt for the evaluation of treatment effects of HIE.
