目的 探讨可吸收缝合线在移植肌腱内的降解吸收机制及移植腱束之间的愈合情况。方法 15只成年新西兰大白兔,切取半腱肌,折叠成双股后,采用3个0快薇乔可吸收缝合线编织缝合,建立采用可吸收缝线编织缝合的前交叉韧带自体肌腱移植重建模型。术后观察实验动物一般情况,术后2、4、8周取材行大体及组织学观察。结果 术后动物肢体活动情况良好,大体观察实验动物前交叉韧带连续性完整,张力适中。组织学观察术后2周位于移植肌腱内部的缝合线无降解,少量炎性细胞浸润,而位于移植肌腱边缘的缝合线开始出现降解,移植腱束间通过纤维组织连接;术后4周移植肌腱内部缝合线部分降解,降解区未见新生组织长入;术后8周移植肌腱内部缝合线完全降解,形成一不规则无染色区。结论 可吸收缝合线在移植肌腱内部降解速度不变,但吸收周期明显延长。

목적 탐토가흡수봉합선재이식기건내적강해흡수궤제급이식건속지간적유합정황。방법 15지성년신서란대백토,절취반건기,절첩성쌍고후,채용3개0쾌미교가흡수봉합선편직봉합,건립채용가흡수봉선편직봉합적전교차인대자체기건이식중건모형。술후관찰실험동물일반정황,술후2、4、8주취재행대체급조직학관찰。결과 술후동물지체활동정황량호,대체관찰실험동물전교차인대련속성완정,장력괄중。조직학관찰술후2주위우이식기건내부적봉합선무강해,소량염성세포침윤,이위우이식기건변연적봉합선개시출현강해,이식건속간통과섬유조직련접;술후4주이식기건내부봉합선부분강해,강해구미견신생조직장입;술후8주이식기건내부봉합선완전강해,형성일불규칙무염색구。결론 가흡수봉합선재이식기건내부강해속도불변,단흡수주기명현연장。
Objective To observe the degradability of absorbable sutures in tendon grafts and the histology of tendon healing in a rabbit model.Methods Semitendinosus tendons were harvested from 15healthy adult New Zealand rabbits in this study. after two-band braiding, the semitendinosus tendons were sutured with the Vicryl Rapide absorbable surgical sutures. The anterior cruciate ligaments (ACL) of bilateral knees were reconstructed with the two-brand semitendinosus tendons in each rabbit. Specimens were collected at 2, 4, 8 weeks after surgery respectively for histological observation.Results Gross observation showed fine motion of the animal limbs and fine condition of the ACL reconstructed after surgery. The absorbable sutures inside the tendon grafts were not degraded and surround by a few inflammatory cells, but the sutures at the edge of tendon graft began to degrade and healing of the tendon grafts was observed at 2 weeks after implantation. The absorbable sutures inside the tendon grafts were partially degraded but newborn tissue was not observed in the degrading region at 4 weeks after implantation. The absorbable sutures were completely degraded and the degradation region was blank at 8 weeks after implantation.Conclusion The speed of degradation of absorbable sutures inside the tendon grafts trends to be stable and the speed of absorption is obviously prolonged.
