目的 探讨内皮素受体拮抗剂(ET-RA)对重症急性胰腺炎(SAP)大鼠心肌损伤的治疗作用.方法 将30 只SD大鼠随机分成3 组:重症急性胰腺炎组(SAPG):胰管逆行注射5%牛黄胆酸钠的方法建立大鼠SAP模型;正常对照(CG)组:处理方法同上,改推注同体积的生理盐水;ET-1 受体拮抗剂治疗组(ET-RAG):SAP 成模后6h腹腔内注射ET-RA,对照组与SAPG注射等体积生理盐水.观察24 h后取标本,分别测定各组血浆淀粉酶(AML)、激酸激酶(CK)及其同工酶(CK-MB)、乳酸脱氢酶-1(LDH1)、ET-1、TNF-α的变化.结果 AML、CK-MB、CK、LDH-1、ET-1、TNF-α在SAPG及ET-RAG 均显著高于正常对照组(P<0.01),而ET-RAG中以上指标的浓度亦显著低于SAPG(P<0.05).结论 ET-RA对SAP 时的心肌损伤有较好的治疗作用.

목적 탐토내피소수체길항제(ET-RA)대중증급성이선염(SAP)대서심기손상적치료작용.방법 장30 지SD대서수궤분성3 조:중증급성이선염조(SAPG):이관역행주사5%우황담산납적방법건립대서SAP모형;정상대조(CG)조:처리방법동상,개추주동체적적생리염수;ET-1 수체길항제치료조(ET-RAG):SAP 성모후6h복강내주사ET-RA,대조조여SAPG주사등체적생리염수.관찰24 h후취표본,분별측정각조혈장정분매(AML)、격산격매(CK)급기동공매(CK-MB)、유산탈경매-1(LDH1)、ET-1、TNF-α적변화.결과 AML、CK-MB、CK、LDH-1、ET-1、TNF-α재SAPG급ET-RAG 균현저고우정상대조조(P<0.01),이ET-RAG중이상지표적농도역현저저우SAPG(P<0.05).결론 ET-RA대SAP 시적심기손상유교호적치료작용.
Objective To explore the effect of treatment of endothelin receptor antagonist (ET-RA) on heart injury in severe acute pancreatitis(SAP) rats. Methods Thirty SD rats were randomly allocated into 3 groups: SAP group (SAPG): Main pancreatic duct was perfused with Sodium Cholate(5%) to induce SAP; Control group (CG): To do the same as that of SAP group, except with normal saline; ET-RA group (ET-RAG): Rats were treated with intraperitoneal injection of ET-RA after the models of SAP were done for 6 hours, while rats of the other two groups were treated with intraperitoneal injection of normal saline. Serum AML, creatine kinase (CK), CK-MB, lactate dehydrogenase 1(LDH1), ET-1 and TNF-α were measured after the models of SAP were done for 24 h. Results The levels of serum AML, CK, CK-MB, LDH1, ET-1 and TNF-α in SAPG and ET-RAG were significantly increased as compared with CG (P<0.01). The levels of all indexes in ET-RAG were decreased than that in SAPG (P<0.05). Conclusion ET-RA have certain effect of treatment on heart injury in SAP rats.
