目的 探讨经耻骨联合上单孔腹腔镜阑尾切除术的安全性、可行性及临床应用价值.方法 回顾性分析太原市中北大学附属医院普外科20例急性阑尾炎患者行经趾骨联合上单孔腹腔镜手术的临床资料.手术均通过经耻骨联合上方阴毛区域内皮肤的一长2.0～2.5 cm的切口内实施,3个穿刺鞘均通过同一切口在不同的筋膜位置分别置入腹腔,按照标准的多孔腹腔镜阑尾切除术方式实施手术.结果 手术均获成功,手术时间50～110 min,平均60 min.术后24 h患者恢复流质饮食,3～5 d出院,无术后出血、肠漏、切口感染等并发症.术后复查一线形切口瘢痕隐藏于耻骨联合上方阴毛区域皮肤上,不易察觉,患者对美容效果满意.结论 临床经验表明经耻骨联合上单孔腹腔镜阑尾切除术安全可行,该技术易学习掌握,不受新器械设备的限制,亦不明显增加费用,具有明显的美容效果.

Objective To evaluate the safety and feasibility of the laparoscopic appendectomy that to be performed through a single incision within the pubic hairline medially at the suprasymphysary area.Methods The clinical data of 20 patients with acute appendicitis underwent suprasymphysary singleincision laparoscopic appendectomy were retrospectively analyzed.The entire procedure was performed through a suprasymphysary single incision measuring 2.0～2.5 cm within the pubic hairline medially at the suprasymphysary area.Three trocars werc insertcd through separate fascial sites within the same skin incision.The appendectomy procedures were then performed in the standard fashion described in multi-port laparoscopic appendectomy.Results Twenty patients successfully underwent suprasymphysary single-incision laparoscopic appendectomy.The operation time was 50～110 min,and mean operation time was 60 min.The patients were discharged from hospital in three to five days after operation.All patients recovered rapidly without any complications such as postoperative intraperitoneal hemorrhage,intestinal fistula,infection of incisional wound and so on.Postoperative reexamination of the incision showed that an inconspicuous linear incisional scar was concealed within the re-growing pubic hair,and all patients were satisfied with the cosmetic results.Conclusion The initial experience with suprasymphysary single-incision laparoscopic appendectomy demonstrates its safety and feasibility,the technique easily taught and performed without the restrictions of new equipment or added cost,the clear advantage is its cosmetic benefit.
