目的 探讨院内综合康复训练对精神分裂症患者认知功能的影响.方法 将80例恢复期精神分裂症患者随机分为研究组40例(脱落2例)和对照组40例(脱落1例),两组均接受常规治疗和护理,研究组在常规治疗护理的基础上参加生活行为技能训练、药物管理技能训练、职业技能训练和社会交往能力训练等院内综合康复训练,观察12周,于训练前后分别采用韦氏成人记忆表修订本(WMS-RC)、精神分裂症阳性症状量表和阴性症状量表(PANSS)、住院患者观察量表(NOSIE)测定,比较两组认知功能的变化情况.结果 入组前两组WMS-RC、PANSS、NOSIE因子评分的比较差异均无统计学意义(P>0.05);训练后12周两组WMS-RC因子中计算评分(15.09±1.57 vs 13.36土1.55,t:3.005,P=0.04)、数字广度评分(13.32±1.68 vs 12.08±1.79,t=3.124,P=0.003)、数字符号评分(8.17±2.21 vs 8.24±1.41,t=3.212,P=0.003)、因子总分(32.79±6.14 vs 29.78±4.58,t=3.412.P=0.001)、记忆商数评分(116.25±3.94 vs 112.72±3.07,t=4.382,P=0.000)比较差异均有统计学意义;PANSS因子(除阳性症状外)、NOSIE因子评分比较差异均有统计学意义(P<0.01或P<0.05).结论 院内综合康复训练能改善精神分裂症患者认知功能.

Objective To investigate the effect of in-hospital comprehensive rehabilitation training schizophrene's cognitive function.Methods 80 schizophrenic patients were randomly divided into study group (n=40)and control group(n=40).Both groups received conventional treatment and care,the study group also received in-hospital comprehensive rehabilitation training such as life skills training,medication management skills training,vocational skills training and social communication traning for 12 weeks.Their cognitive function was measured with Wechsler Adult Memor(Revised)(WMS-RC),Positive and Negative Symptom Scale(PANSS),Nurses'Observation Scale for Inpatient Evaluation(NOSIE)before and after the training,respectively,and changes in cognitive function were compared.Results There was no statistically significance in WMS-RC,PANss,NOSIE scores between the study group and the cantrol group before training.There was statistically significance in WMS-RC,Calculation scores(15.09±1.57 vs 13.36±1.55,t=3.005.P=0.04),dtgit span scores(13.32±1.68 vs 12.08±1.79,t=3.124,P=0.003),number sign(8.17±2.21 vs 8.24±1.41,t=3.212,P=0.003),total sares(32.79±6.14 vs 29.78±4.58,t=3.412,P=0.001),memory quotient sares(116.25±3.94 vs 112.72±3.07,t=4.382,P=0.000)between the two groups ccfter 12 weeks;There was statistically signifieamce in PANSS,NOSIE scores between the study group and the control group after training 12 weeks.Conclusions In-hospital comprehensive rehabilitation training can improve schizophrene's cognitive function.
