目的研究硬膜外阻滞前后妊高征患者血浆中一氧化氮(NO)和内皮素(ET)的变化。方法选择20例择期剖宫产患者,正常妊娠组(NLP组)10例,妊高征组(PIH组)中度妊高征患者10例,分别于硬膜外阻滞前及阻滞完善后抽取母体静脉血,测定血浆一氧化氮和内皮素的浓度。结果阻滞前PIH组血浆NO浓度明显低于NLP组(P<0.01),ET含量显著高于NLP组(p<0.01);阻滞后两组血浆NO水平均显著升高(P<0.05),ET水平明显降低(P<0.05)。结论血浆中NO含量降低和ET浓度增高是妊高征发病的一个重要环节,硬膜外阻滞可使妊高征患者血浆NO水平和ET浓度降低,是一有效和安全的麻醉方法。

목적연구경막외조체전후임고정환자혈장중일양화담(NO)화내피소(ET)적변화。방법선택20례택기부궁산환자,정상임신조(NLP조)10례,임고정조(PIH조)중도임고정환자10례,분별우경막외조체전급조체완선후추취모체정맥혈,측정혈장일양화담화내피소적농도。결과조체전PIH조혈장NO농도명현저우NLP조(P<0.01),ET함량현저고우NLP조(p<0.01);조체후량조혈장NO수평균현저승고(P<0.05),ET수평명현강저(P<0.05)。결론혈장중NO함량강저화ET농도증고시임고정발병적일개중요배절,경막외조체가사임고정환자혈장NO수평화ET농도강저,시일유효화안전적마취방법。
Objective This investigation was designed to evaluate the effects of epidural block on plasma nitric oxide (NO) and endothelin (ET) concentrationns in the pregnancy-induced hypertensive patients. Methods 10 normal late-stage gravies having undergone elective cesarean section served as normalpregnancy group (NLP group) and 10 moderate pregnancy-induced hypertension gravies (PIH) as PIH group. The intravenous blood samples were taken immediately before epidural block and after epilural block completely to determine the plasma concentration of NO and ET. R~ Before epidural block, the plas ma NO level significantly lower and ET level higher in PIH group than those in NIP group(P <0.01).The plasma NO contents increased and ET concentration reduced significantly after epidural block in both groups. Conclnsions It is an important factor for the cause of pregnancy-induced hypertension that plasma NO level decreases and ET level increases. Epidural block can improve plasma NO level and decrease plasma ET content in the patients with pregnancy-induced hypertension and is an useful and safe anesthesia method for these patients.
