目的 探讨超声弹性成像在亚急性甲状腺炎( subacute thyroiditis,SAT)病程分期中的应用价值.方法 将65例共116个病灶分为急性期组(Ⅰ组)、中期组(Ⅱ组)、恢复期组(Ⅲ组).各组病灶均接受常规超声及弹性超声检查并进行组间比较,动态观察分析全部病例两种检查方法的声像图变化规律,其中23例获得完整资料.结果 ①常规超声显示病灶回声以Ⅲ组最高,Ⅰ组回声明显低于Ⅱ及Ⅲ组(P<0.01).CDFI显示Ⅰ组血流信号以0级为主,Ⅱ组较Ⅰ组血流明显增多(P<0.01),Ⅲ组血流多恢复正常.②Ⅰ组弹性评分最高,随着病程进展,评分逐渐减低,组间比较差异有统计学意义(P<0.01).③Ⅰ组弹性应变比值(strain ratio)最高,与Ⅱ、Ⅲ组比较差异有统计学意义(P<0.01),Ⅲ组最低.结论 超声弹性成像能够简便、无创、半定量地反映SAT的硬度信息,为其诊断及分期诊断提供了一项新的检测方法.

Objective To access the clinical value of elastosonography by summarizing the characteristics of conventional ultrasound and elastosonography of subacute thyroiditis (SAT) in different stages.Methods One hundred and sixteen SAT lesions detected from sixty-five patients were enrolled in the study and divided into three groups,including acute group (group Ⅰ ),medium group (group Ⅱ ) and recovery group (group Ⅲ ).All the patients accepted conventional ultrasound and elastosonography examinations.Results were compared by ANOVA and Kruskal-Wallis H test.Twenty-three patients were successfully followed.Results Among the three groups,echo intensity of group Ⅲ was the highest and the echo intensity of group Ⅰ was obvious lower ( P <0.01).CDFI showed that most of the blood supply grading of group Ⅰ was zero,while blood supply of group Ⅱ was much richer ( P <0.01).Blood signals in group Ⅲ became normal.Both the elastographic score and the value of strain ratio decrease along with the course of the disease.Differences among the groups were statistically significant.Conclusions Elastosonography was a simple,noninvasive tool in diagnosing SAT by semi-quantitatively demonstrate the stiffness of the lesions.
