目的总结角结膜原位癌的临床特征,探讨其诊断、不同治疗方法对患者复发及预后的影响.方法对2002年1月至2006年12月收治的14例诊断为角结膜原位癌的住院病例的临床特征、诊断、手术方式及随访结果进行回顾性分析.结果发病没有性别及左右眼的差别,50%的肿瘤发生在角巩缘,术前诊断为原位癌的准确率为42.8%,治疗方法以手术切除为主,辅以角膜移植、羊膜移植、异体结膜移植、冷冻的方法.随访2～6年,术后复发率为7%.结论提高早期诊断率,对于改善预后,预防复发起着重要作用.相对安全有效的治疗方法可有多种,包括距肿瘤边缘2mm手术切除,辅以异体角结膜移植、羊膜移植及冷冻.

Objective To summarize the clinical characteristics of corneal and conjunctival carcinoma in situ,and explore the influence of diagnosis and different therapeutic methods to recurrence and prognostic.Methods Fourteen cases diagnosed as corneal and conjunctiva carcinomas in situ in our hospital from January 2002 to December 2006 were analyzed retrospectively.The clinical features,diagnosis,surgical methods and follow-up results were included.Results There was no gender and eye difference.50% carcinomas were observed in the corneal limbus.The fight diagnostic rate before operation was 42.8%.Therapeutic methods were mainly operation,including corneal transplantation,amnion membrane transplantation,conjtmctival transplantation and cryocoagulation.The follow-up time was 2 to 6 years,the recurrence rate was 7%.Conclusions Early diagnosis is important to improve prognostic.There are many choices for effective therapy including operative remove and corneal transplantation,amnion membrane transplantation or cryocoagulation.
