目的研究高分辨螺旋CT对中度哮喘患者气道重塑的评估.方法本研究为单中心开放研究,通过纳入30例中度持续哮喘组和对照组30例行肺功能测试、高分辨螺旋CT扫描.比较对照组、哮喘组的肺功能指标、气道反应性,并与HRCT测试的气道壁厚度(WT)和气道壁相对面积(WA%)值做相关分析.结果哮喘患者气道壁厚度(WT)较对照组增厚,相对气道壁面积(WA%)增加;WT和WA%与受试者气道高反应性正相关,而与FEV1/FVC、FEV1%呈负相关.近端气道相关程度强于远端气道.结论高分辨螺旋CT影像能有效评估哮喘患者的气道重塑,并与哮喘患者症状、肺功能检查密切相关,可用于评估哮喘的严重性、分型和哮喘患者的预后,评定治疗效果和随访观察.

Objective To investigate the role of high-resolution CT(HRCT) in evaluating the degree of airway remode-ling in the moderate asthmatic patients. Methods A single-center,open-label study was conducted in adult patients with moder-ate asthma. 30 asthmatic patients and 30 control subjects were enrolled. All the subjects were required to take pulmonary function test and HRCT scan. Statistic methods were used to compare the pulmonary function test result and airway responsiveness between asthmatic group and control group. The relationship between pulmonary function test,airway responsiveness and WT,WA% which measured by HRCT were also analyzed. Results The value of WT and WA% in asthmatic group was higher than that of control group. WT and WA% had a positive correlation with airway hyper responsiveness and a negative correlation with FEV1/FVC, FEV1% pre. Conclusion HRCT image could evaluate the airway remodeling in asthmatic patients effectively and it have a close relationship with asthmatic symptom and pulmonary function. It can be used to assess the severity,prognosis of asthma,and evalu-ate the efficiency of treatment.
