目的探讨谷氨酰胺(Gln)强化的TPN对胃肠道肿瘤术后病人营养状况及免疫功能的影响。方法 20例胃肠道肿瘤病人术后随机分为常规TPN组（对照组）和(TPN+Gln)组（研究组），治疗共8d，观察血清前白蛋白、血清转铁蛋白、氮平衡变化。并检测其前后外周血lgG、lgM、lgA、C3、C4的变化。结果（1）两组病人呈正氮平衡趋势，研究组与对照组比较差异明显（P<0.01）。两组病人血清蛋白均上升，研究组回升更明显，与对照组比较具有显著差异（P<0.01）。（2）研究组外周血lgG、lgM、lgA明显升高，与对照组比较差异显著（P<0.05）。研究组C3、C4明显升高与对照组比较差异显著（P<0.05）。结论 Gln强化的TPN提高了肠外营养支持的效果，改善了胃肠道肿瘤术后的免疫功能。
목적 탐토곡안선알(Gln)강화적TPN대위장도종류술후병인영양상황급면역공능적영향。방법 20례위장도종류병인술후수궤분위상규TPN조（대조조）화(TPN+Gln)조（연구조），치료공8d，관찰혈청전백단백、혈청전철단백、담평형변화。병검측기전후외주혈lgG、lgM、lgA、C3、C4적변화。결과 （1）량조병인정정담평형추세，연구조여대조조비교차이명현（P<0.01）。량조병인혈청단백균상승，연구조회승경명현，여대조조비교구유현저차이（P<0.01）。（2）연구조외주혈lgG、lgM、lgA명현승고，여대조조비교차이현저（P<0.05）。연구조C3、C4명현승고여대조조비교차이현저（P<0.05）。결론 Gln강화적TPN제고료장외영양지지적효과，개선료위장도종류술후적면역공능。
Objective To investigate the effect of glutamine-intensified TPNon nutritional state and immunofunction in the postoperative patients with gastrointestinal tract tumour.Methods 20 patients were divided into two groups including standard TPN group (control group,10 cases) and Glutamine-intensified TPN group(study group,10 cases). Period of treatment was 8 days.Immunofunction(IgG,IgM,IgA,C3,C4), serum albumin , nitrogen balance were tested.Results （1）The patients serum albumin in the study group was increased significantly as compared with control group (P<0.01) All patients have positive nitrogen balance.（2）The patients’immunofunction (IgG,IgM,IgA,C3,C4) in the study group was increased significantly than control group (P<0.05).Conclusion The treatment of Glutamine-intensified TPN can improve the immunofunction and the effect of parenteral nutrition on the patients with gastrointestinal tract tumour after operation.
