目的 观察早期应用冬眠合剂对严重烫伤大鼠肺损伤是否具有保护作用. 方法建立Ⅲ度烫伤大鼠模型,随机分为冬眠合剂组和对照组(未用冬眠合剂),每组36只.分别在伤后3、5、7、10 d测定大鼠动脉血氧分压,检测肺组织中过氧化物酶(MPO)活性、丙二醛(MDA)含量、肿瘤坏死因子α(TNF-α)及γ干扰素(IFN-γ)蛋白表达,动态观察伤后肺组织病理学变化. 结果与对照组大鼠伤后3 d动脉血氧分压(8.86±0.23)kPa(1 kPa=7.5 mm Hg)比较,冬眠合剂组该时相点明显升高[(12.58±0.41)kPa,P<0.01].冬眠合剂组大鼠伤后各时相点肺组织MPO活性和MDA含量下降(P<0.05或P<0.01);伤后3、5、7 d肺组织TNF-α表达显著下调(P<0.05或P<0.01),伤后5、7、10 d肺组织IFN-γ的表达亦显著下调(P<0.01).冬眠合剂组大鼠肺泡间质水肿减轻,炎性细胞浸润减少. 结论大鼠烫伤后及时复苏并给予冬眠合剂,能适度抑制机体应激反应并下调促炎因子表达,改善早期肺功能.

목적 관찰조기응용동면합제대엄중탕상대서폐손상시부구유보호작용. 방법건립Ⅲ도탕상대서모형,수궤분위동면합제조화대조조(미용동면합제),매조36지.분별재상후3、5、7、10 d측정대서동맥혈양분압,검측폐조직중과양화물매(MPO)활성、병이철(MDA)함량、종류배사인자α(TNF-α)급γ간우소(IFN-γ)단백표체,동태관찰상후폐조직병이학변화. 결과여대조조대서상후3 d동맥혈양분압(8.86±0.23)kPa(1 kPa=7.5 mm Hg)비교,동면합제조해시상점명현승고[(12.58±0.41)kPa,P<0.01].동면합제조대서상후각시상점폐조직MPO활성화MDA함량하강(P<0.05혹P<0.01);상후3、5、7 d폐조직TNF-α표체현저하조(P<0.05혹P<0.01),상후5、7、10 d폐조직IFN-γ적표체역현저하조(P<0.01).동면합제조대서폐포간질수종감경,염성세포침윤감소. 결론대서탕상후급시복소병급여동면합제,능괄도억제궤체응격반응병하조촉염인자표체,개선조기폐공능.
Objective To investigate the protective effect of inhibition of stress (lytic cocktail) on lung injury in severe bum rats at early stage. Methods Sprague-Dawley rats inflicted with 30% TBSA full- thickness burn were randomly divided into A group ( n = 36, fluid resuscitation with administration of lyric cocktail) , B group( n = 36, fluid resuscitation only). Lung function was evaluated by partial pressure of ox- ygen ( PaO2 ) in arterial blood and histopathologic changes on 3, 5, 7, 10 post bum day (PBD). The levels of malonyldialdehyde (MDA) , myeloperoxidase (MPO) , tumor necrosis factor-α(TNF-α) and interferon-γ (IFN-γ) in lung tissue were measured at the same time points. Results The PaO2 level in A group on 3 PBD (12.58±0.41 kPa) was significantly higher than that in B group ( 8.86±0. 23 kPa, P < 0.01 ). Compared with those in B group, the levels of MDA and MPO were significantly decreased in A group at each time point( P <0.05 or0.01) ,the levels of TNF-α on 3,5,7 PBD ( P <0.05 or0.01)and IFN-γ on 5,7,14 PBD( P < 0. 01 ) were also decreased in A group. Swollen lung mesenchyme was alleviated,infiltra- tion of inflammatory cell was lessened in A group. Conclusion Lytic cocktail combined with immediate fluid resusitation can inhibit stess response, downregulate the expression of inflammatory factor, ameliorate lung function in severe burn rat at early stage.
