目的探讨大切口无缝线白内障囊外摘除术(SLIMCE)后泪膜情况.方法随机选择行SLIMCE术眼68例,分别于术前及术后7、14、30、60、90d行症状、泪膜破裂时间(BUT)、基础泪液分泌试验(SIt)、角膜荧光素染色(FL)检测.运用结膜印迹细胞学及显微图像分析系统对SLIMCE术区结膜杯状细胞进行记数、分析.结果术后3个月内诉术眼不适患者较术前增加.术后2周内SIt增加,1个月内BUT缩短、FL增高,与术前相比有统计学意义(P<0.05).结论术后一段时期内,SLIMCE会影响泪膜稳定性,应注意术前泪膜功能检测及术中、术后对泪膜的保护.

Objective To investigate the alterations of tear film after sutureless large incision manual cataract extraction (SLIMCE). Methods SAw-eight eyes undergone SLIMCE operation were chosen ran-domly. Slit-limp microscope, break- up time (BUT), schirmmer Ⅰ test (Sit), FL were used to observe the alter-ations of tear film at different time points post-opcration. Impression cytology and microphoto-analyses tech-niques were also applied to observe the goblet cells at different time points post-operation. Results Subjective complaint of dry eye within 3 months after the operations were significantly increased compare with pre-opera-tions. The schirmmer Ⅰ test was greatly increased 2 weeks post operation,the mean scores of fluorescence in-creased and the mean break-up time decreased 1 month post operation (P <0.05 ). The impression cytology de-tection showed that the number of conjunetiva goblet cells in surgery area is lower at different time point post-operation respectively (29.62±4.31 ; 22.69±4.71 ; 39.05±4.27; 44.83±3.63; 45.76±3.55)than that in pre-operation (46.35±4.86 ) (P <0.05 ). Conclusion SLIMCE operation has effect on the stability of tear film.
