目的 探讨免疫共刺激分子4-1BB/4-1BBL在过敏性紫癜(HSP)患儿外周血单个核细胞(PBMC)中的表达及其意义. 方法 34例仞发HSP患儿以临床表现有无明显肾脏损害分为紫癜性肾炎(HSPN)组13例,过敏性紫癜无肾脏受累(NO-HSPN)组21例,同期体检健康儿童20例为正常对照组.应用流式细胞仪(FCM)检测3组入选者PBMC中4-1BB和4-1BBL的蛋白表达水平. 结果 HSP患儿4-1BB[(1.32±0.51)%、(1.36±0.62)%]、4-1BBL[(6.07±1.67)%、(7.14±1.94)%]蛋白的表达明显高于正常对照组[(0.70±0.25)%、(3.64±1.28)%],差异均有统计学意义(P<0.01).加入植物血凝素(PHA)刺激后,3组4-1BB、4-1BBL蛋白水平的表达均明显增高[(46.54±8.19)%比(1.32±0.51)%,(50.31±5.17)%比(1.36±0.62)%;(16.31±3.80)%比(6.07±1.67)%,(17.79±2.73)%比(7.14±1.94)%;均P<0.01],且NO-HSPN及HSPN组较正常对照组升高更明显.加入地塞米松后,HSP患儿4-1BB、4-1BBL蛋白水平的表达均明显减少[(0.62±0.29)%比(1.32±0.51)%,(0.68±0.33)%比(1.36±0.62)%;(3.06±1.33)%比(6.07±1.67)%,(3.71±1.27)%比(7.14±1.94)%;均P<0.01],正常对照组的表达减少不明显. 结论 HSP患儿PBMC共刺激分子4-1BB、4-1BBL蛋白水平的表达异常增高,说明4-1BB、4-1BBI这一对共刺激分子可能参与HSP的起病,提示两者可以作为监测HSP患儿病情以及检测早期肾脏损害的免疫学指标.

Objective To investigate the expression of costimulatory molecules 4-1BB and 4-1BBL in patients with Henoch-Schonlein purpura(HSP)and explore the possible significance. Methods Thiroy-four HSP patients as research objects were devided into two groups:the first group is 13 Henoch-Schonlein purpura nephritis (HSPN)patients,the second is 21 NO-HSPN patients and the concrol is 20 healthy children.Applying flow cytometry(FCM)technique and phytohaemagglutinin(PHA),dexamethasone(DEX)as intervention to quantitatively analyze 4-1BB and 4-1BBL expression in PBMC(peripheral blood mononuclear cells)of 34 HSP patients. Results Compaired with normal controls,the expression of 4-1BB,4-1BBL positive expression cell rate(PECR)increase obviously in HSP especially in HSPN.The expression of PECR in all objections both increase obviously especially in HSP after add in PHA(P <0.01).The expression of 4-1BB,4-1BBL PECR of the control in PBMC decrease little but decrease obviously in NO-HSPN and HSPN after add in Dex(P < 0.01). Conclusions The abnormal expression of 4-1BB and 4-1BBL may play a critical role in the pathogenesis and development of HSP.4-1BB and 4-1BBL may be immunological marker of evaluating the condition of patients with HSP.
