目的 探讨64排螺旋CT用于冠状动脉成像的图像质量和诊断冠状动脉狭窄≥50%的准确性.方法 采用64排螺旋CT对临床拟诊为冠状动脉性心脏病的113例病例扫描,其中42例有冠状动脉造影与64排螺旋CT冠状动脉成像作为对照.分析冠状动脉成像质量和计算诊断冠状动脉狭窄≥50%的敏感性、特异性、阳性预测价值及阴性预测价值.结果 成像质量Ⅰ级93例(82.30%),Ⅱ级17例(15.04%),Ⅲ级3例%(2.65%).64螺旋CT冠状动脉成像诊断冠状动脉狭窄≥50%的准确性为92.86%,敏感性、特异性、阳性和阴性预测价值分别为95.29%、93.33%、96.15%、87.50%.结论 64排螺旋CT冠状动脉成像图像质量高,对冠状动脉狭窄≥50%有较高的诊断价值.

Objective To evaluate the quality of coronary images of 64-slice CT and the diag
nostic accuracy of coronary artery stenosis ≥ 50%. Methods One hundred and thirteen patients with
suspected coronary artery disease were analyzed with 64-slice CT. Among them, 42 patients underwent
conventional coronary angiography ( CAG ). Image quality was analyzed, and the sensitivity, specificity,
positive and negative predictive value of ≥ 50% stenosis were also calculated. Results The images were
excellent ( Ⅰ, 93, 82.30% ), sufficient ( Ⅱ, 17, 15.04% ) andnon-diagnostic ( Ⅲ, 3, 2.65% ),
respectively. The sensitivity, specificity, positive and negative predictive value of ≥ 50% stenosis were 95.
29%, 93.33%, 96.15%, 87.50%, respectively. Conclusion Coronary images from 64-slice CT have
excellent quality. The 64-slice CT angiography provides high diagnosis value in detecting coronary artery
stenosis ≥ 50%.
