目的 探讨瘦素(Leptin)、脂联素(Adi)、胰岛素(INS)及血脂、血糖(GLU)与脑血管病的相互关系,评价Leptin、Adi在脑血管病发病中的作用.方法 选136例不同类型脑血管病患者和39例健康人,采用放射免疫分析法测定空腹血清Leptin、Adi水平,电化学发光免疫分析法测定血清INS水平,生化分析仪测定血脂、血糖水平,并进行相关分析.结果 脑血管病患者血清Leptin水平明显高于健康对照组;血脂、GLU、INS水平类似Leptin变化,Adi显著低于健康对照组.相关分析显示,Leptin水平与INS、TG、TC及GLU呈显著正相关(r=0.427,P<0.01;r=0.289,P<0.05;r=0.387,P<0.01;r=0.263,P<0.05);Adi水平与上述各项指标的相关性呈显著负相关(P<0.01～0.05).结论 脑血管病患者的血清Leptin、Adi水平与脂糖代谢、高INS血症以及代谢综合征的其他成分密切相关,提示高Leptin、高INS血症及低Adi水平可能是脑血管病的又一特征,并在其发病中起重要作用.

Objective To investigate the correlation of cerebrovascular disease with Leptin,Adi,INS,blood lipid and GLU.Evaluate the role of Leptin and Adi in the cause of cerebrovascular disease.Methods 136 patients with different types of cerebrovascular disease and 39 healthy people are selected.Fasting serum Leptin and Adi are detected with Radio Immunoassay.Serum INS is detected with Electrochemical Luminescence Immunoassay Analyzer.The level of blood lipid is detected by Biochemical Analyzer.The correlation of the above detected items is analyzed.Results The levels of blood lipid,GLU and INS were similar to that of Leptin whose level was higher in the patients with cerebrovascular disease than the level of the cases in the control group.The level of Adi was significant higher in the control group than that in the patients group.Correlation Analysis indicate,Obviously,the value of Leptin was positively correlated with the value of INS,TG,TC and GLU(r=0.427,P<0.01;r=0.289,P<0.05;r=0.387,P<0.01;r=0.263,P<0.05).The value of Adi was negatively correlated with the value of the above-mentioned items(P<0.01-0.05).Conclusion The levels of Leptin and Adi of patients with cerebrovascular disease are closely related with Glucose metabolism in lipid,high-INS hyperlipemia and other parts of metabolic syndrome.High levels of Leptin and INS hyperlipidemia,low levels of Adi maybe another characteristics of cerebrovascular disease.They may play important roles in the cause of the cerebrovascular disease.
