目的探讨食管鳞状上皮低级别上皮内瘤变自然演变的规律及其在食管癌形成中的作用.方法选择我院2001年3月-2004年3月42例胃镜检查确诊为食管鳞状上皮低级别上皮内瘤变患者,对其随访至2009年3月,分析其病理变化规律.结果内镜下病变消失8例,保持低级别上皮内瘤变22例,加重至高级别上皮内瘤变或鳞癌12例.不同病变范围的低级别上皮内瘤变患者,其随访结果间差异无统计学意义(P>0.05).不同胃镜分型的低级别上皮内瘤变患者,其随访结果间差异有统计学意义(P<0.05);两两比较结果:红斑型与隆起型、糜烂型与隆起型、红斑型与浅溃疡型、糜烂型与浅溃疡型比较,差异均有统计学意义(P<0.05).结论食管鳞状上皮低级别上皮内瘤变有一定的癌变能力,与胃镜分型相关,应密切随访.

Objective To explore the regularity of natural development of esophageal low-grade intraepithelial neoplasia and its role in formation of esophageal cancers.Methods Forty-two esophageal low-grade intraepithelial neoplasia patients confirmed by esophageal endoscopy from March 2001 to March 2004 were followed up till March 2009,and the regularity of its pathological changes was analyzed.Results Under endoscopy,lesions disappeared in 8 patients,low-grade intraepithelial neoplasia remained in 22,aggravated to high-grade or squamous cell carcinoma in 12.There was not significant difference in follow-up result between low-grade intraepithelial neoplasia patients in varying lesion ranges (P>0.05),but there was between patients in different endoscopic typings (P<0.05),and significant difference was noted between types erythema and elevated,erosive and elevated,erythema and shallow ulcer,erosive and shallow ulcer (P<0.05).Conclusion Esophageal low-grade intraepithelial neoplasia,being of some canceration ability,is related to endoscopy typing.And patients need close follow-ups.
