目的 探讨三种炎症因子超敏C反应蛋白(hsCRP)、白细胞介素-6(IL-6)、肿瘤坏死因子(TNF-α)与糖尿病前期发生的相关性.方法 94名35~70岁志愿者被纳入研究,其中61名糖尿病前期者和33名正常血糖者.对纳入研究者进行身高、体重、腰围、臀围、血压测量;计算BMI、腰臀比;行口服糖耐量试验(CGTT);检测血脂、尿酸、hsCRP、IL-6,TNF-α等.比较糖尿病前期组与正常血糖组hsCRP、IL-6、TNF-α及上述代谢指标水平的差异,并分析hsCRP、IL-6、TNF-α与其他代谢指标的相关性.结果 糖尿病前期组hsCRP(2.17±0.39)mg/L、IL-6(3.11±0.66)pg/ml、TNF-α (3.10±0.37) pg/ml,均高于正常血糖组hsCRP (1.99±0.42)mg/L、IL-6(2.82±0.47) pg/ml、TNF-α (3.09±0.28) pg/ml,两组比较hsCRP、IL-6差异均具有统计学意义(P<0.05);相关分析显示hsCRP水平与体重、腰围、臀围、腰臀比、OGTT2h血糖、甘油三酯、尿酸呈正相关.逐步回归分析显示影响hsCRP(y1)水平的主要因素为腰臀比(a1)、甘油三酯(b1)、收缩压(c1)、舒张压(d1)、肿瘤坏死因子(e1)、OGTT2/b时血糖(f1).回归方程:y1=-5.428+3.507a1+0.068bh10.009c1 + 0.009d1+0.21le1+0.033f1.在调整年龄、性别等影响因素后,hsCRP(OR=3.715,P<0.05)、IL-6(OR=2.647,P<0.05)与糖尿病前期的发生相关.结论 糖尿病前期者hsCRP、IL-6水平高于正常血糖者.hsCRP、IL-6、TNF-α水平的升高可能是发生糖尿病前期的危险因素.

목적 탐토삼충염증인자초민C반응단백(hsCRP)、백세포개소-6(IL-6)、종류배사인자(TNF-α)여당뇨병전기발생적상관성.방법 94명35~70세지원자피납입연구,기중61명당뇨병전기자화33명정상혈당자.대납입연구자진행신고、체중、요위、둔위、혈압측량;계산BMI、요둔비;행구복당내량시험(CGTT);검측혈지、뇨산、hsCRP、IL-6,TNF-α등.비교당뇨병전기조여정상혈당조hsCRP、IL-6、TNF-α급상술대사지표수평적차이,병분석hsCRP、IL-6、TNF-α여기타대사지표적상관성.결과 당뇨병전기조hsCRP(2.17±0.39)mg/L、IL-6(3.11±0.66)pg/ml、TNF-α (3.10±0.37) pg/ml,균고우정상혈당조hsCRP (1.99±0.42)mg/L、IL-6(2.82±0.47) pg/ml、TNF-α (3.09±0.28) pg/ml,량조비교hsCRP、IL-6차이균구유통계학의의(P<0.05);상관분석현시hsCRP수평여체중、요위、둔위、요둔비、OGTT2h혈당、감유삼지、뇨산정정상관.축보회귀분석현시영향hsCRP(y1)수평적주요인소위요둔비(a1)、감유삼지(b1)、수축압(c1)、서장압(d1)、종류배사인자(e1)、OGTT2/b시혈당(f1).회귀방정:y1=-5.428+3.507a1+0.068bh10.009c1 + 0.009d1+0.21le1+0.033f1.재조정년령、성별등영향인소후,hsCRP(OR=3.715,P<0.05)、IL-6(OR=2.647,P<0.05)여당뇨병전기적발생상관.결론 당뇨병전기자hsCRP、IL-6수평고우정상혈당자.hsCRP、IL-6、TNF-α수평적승고가능시발생당뇨병전기적위험인소.
Objedtive To evaluate the correlation between prediabete and inflammatory factors such as high-sensitivity C-reactive protein (hsCRP),interleukin-6 (IL-6),tumor necrosis factor-α(TNF-α).Methods 94 volunteers (aged between 35 to 70 years) were enrolled in the research,including 61 prediabetes and 33 normal glucose.Anthropometric measurements such as body height,body weight,waist circumference,hip circumference and blood pressure were obtained;BMI,waist-to-hip ratio were calculated.Each subject underwent oral glucose tolerance test.Fasting glucose,OGTT 2 hours glucose,blood-fat,uric acid,highsensitivity C-reactive protein,interleukin-6,tumor necrosis factor-α were detected in them.The levels of inflammatory factors and other metabolic indicators were analyzed between the prediabetes and normal blood glucose.And the correlation between prediabete and inflammatory factors were evaluated.~Results Prediabetes group had higher median levels of hsCRP (2.17±0.39) mg/L,IL-6 (3.11±0.66) pg/ml,TNF-α(3.10±0.37) pg/ml than those with normal blood glucose.There are statistically significant difference between prediabetes and normal glucose among hsCRP,IL-6(P<0.05).Weight,waist circumference,hip circumference,OGTT 2 hours glucose,triglyeeride,uric acid were positively correlated with hsCRP level.Stepwise analysis showed that the main factors of hsCRP (y1) level were waist-to-hip ratio (a1),triglyceride (b1),DBP (c1),SBP(d1),TNF-α(e1).The equation was y1=-5.428+3.507a1+0.068b1-0.009c1+0.009d1+0.211e1+0.033f1.There was a positively correlation between hsCRP level(OR=3.715,P<0.05) and IL-6 level (OR=2.647,P<0.05) and the prediabetes after adjusting the influences of age and gender.Conclusions The levels of hsCRP,IL-6 in prediabetes were higher than those with normal blood glucose and hsCRP,IL-6,TNF-α may be the risk factors of prediabetes.
