目的探讨微通道经皮肾镜取石术一期处理结石性脓肾的方法与效果。方法 2007年1月至2010年8月，共有78例结石性脓肾患者在本院行微通道经皮肾镜取石术，对其临床资料进行回顾性分析。男32例，女46例。年龄21 ～77岁，平均42岁。患者取俯卧位，B超引导下建立工作通道，用钬激光碎石机碎石。结果 78例均一期成功建立皮肾通道，且均为单通道碎石。结石清除率为89.7％。所有患者未输血，且无严重并发症。术后64例获得随访。患肾功能恢复或部分恢复56例，患肾萎缩8例，无患肾切除病例。结论采用一期微通道经皮肾镜取石术治疗结石性脓肾安全有效。
목적 탐토미통도경피신경취석술일기처리결석성농신적방법여효과。방법 2007년1월지2010년8월，공유78례결석성농신환자재본원행미통도경피신경취석술，대기림상자료진행회고성분석。남32례，녀46례。년령21 ～77세，평균42세。환자취부와위，B초인도하건립공작통도，용화격광쇄석궤쇄석。결과 78례균일기성공건립피신통도，차균위단통도쇄석。결석청제솔위89.7％。소유환자미수혈，차무엄중병발증。술후64례획득수방。환신공능회복혹부분회복56례，환신위축8례，무환신절제병례。결론채용일기미통도경피신경취석술치료결석성농신안전유효。
Objectives To evaluate the feasibility and efficacy of mini - percutaneous nephrolithotomy (mini - PCNL) in a single session in treating calculous pyonephrosis. Methods We retrospectively reviewed the records of 78 patients (32 males and 46 females) who underwent mini - PCNL at our center for treatment of calculous pyonephrosis between January 2007 and August 2010. Mean patient age was 42 years ( range 21 to 77 years). Percutaneous access was established under ultrasound guidance with the patient in the prone position. A holmium laser lithotripter was used for in situ lithotripsy. Results The percutaneous access was successfully established in all patients. All of the patients were managed with only one tract. The stone clearance rate for mini - PCNL ( not inclusive of adjunctive shock wave lithotripsy) was 89.7％. All patients did not receive blood transfusion,and there were no major complications. Follow - up of 4 months to 3 years was available in 64 cases. The renal function improved in 56 cases. Renal atrophy was observed in 8 cases. Conclusions Mini - PCNL in a single session is safe and effective in the management of calculous pyonephrosis.
