目的分析原发性肺非霍奇金氏淋巴瘤的CT表现,提高对此病的认识.方法回顾性分析经病理及免疫组织化学证实的肺非霍奇金氏淋巴瘤的CT及临床资料,运用诊断标准对确诊为原发性非霍奇金氏淋巴瘤的11例患者进行分析.结果 CT表现:双肺多发病变3例,单肺多发病变5例,单肺孤立病变3例.其中:肺内肿块型5例,肺炎型4例,既有肺内肿块又有斑片影的2例.8例行增强CT的均表现为轻度均匀强化,病灶内未见明显坏死液化区;伴有胸腔积液的有2例,胸膜增厚的2例.11例患者中有3例行开胸手术,术后病检为非霍奇金氏淋巴瘤;另外8例均行CT导向下穿刺活检行病理及免疫组织化学检测为非霍奇金氏淋巴瘤.结论原发性肺非霍奇金氏淋巴瘤的影像学表现多样,其中以结节肿块型和肺炎实变型最常见,结节肿块周围出现磨玻璃影具有一定特征性.

Objective To study the CT manifestations of primary pulmonary non-Hodgkinp't lymphoma and to improve the rate of successful diagnosis of primary pulmonary non-Hodgkin't lymphoma. Methods The clinical data and CT images of 11 cases with primary pulmonary non-Hodgkin's lymphoma were retrospectively analyzed. All cases were comfirmed by histopathology and immunohistochemistry. Results The manifestations of the 11 cases were as follows:5 cases had multiple lesions of both lungs, 3 cases had multiple lesions of one lung and 3 cases had solitary lesion. 5 cases showed pulmonary mass or nodules; 4 cases showed consolidation and 2 cases had both masses and consolidation. 8 cases showed light well-distributed enhancement after contrast injection. 2 cases had pleural effussion and another 2 cases had pleural thickening. Conclusion The CT manifestations for primary pulmonary non-Hodgkin's lymphoma may be various and nonspecific. Pulmonary mass and consolidation with air bronchogram are common. A few features remain characteristic, such as masses acompanied with ground-glass opacity.
