目的探讨腹腔镜内环口结扎加脐正中襞覆盖在儿童腹股沟疝手术中的应用价值.方法搜集140例腹股沟斜疝患儿,将其随机分为两组:60例采用腹腔镜内环口高位结扎加脐正中襞覆盖疝环口治疗的患儿为腹腔镜组,80例采用传统开放手术治疗的患儿为传统手术组；比较两组手术后恢复情况及随访结果.结果腹腔镜组的手术时间为( 20.8±6.5) min,传统手术组的为(33.5±10.6) min,两组比较差异有显著性(P＜0.01)；术后住院时间腹腔镜组为(2.4±1.5)d,传统手术组为(4.2±1.3)d,两组间差异有显著性(P＜0.01)；腹腔镜组阴囊水肿、疝复发、异时疝发生率均明显少于传统手术组(P＜0.05).结论腹腔镜疝囊高位结扎加脐正中襞内环口覆盖加强腹壁治疗小儿腹股沟疝安全可行,恢复快,不易复发.
목적 탐토복강경내배구결찰가제정중벽복개재인동복고구산수술중적응용개치.방법 수집140례복고구사산환인,장기수궤분위량조:60례채용복강경내배구고위결찰가제정중벽복개산배구치료적환인위복강경조,80례채용전통개방수술치료적환인위전통수술조；비교량조수술후회복정황급수방결과.결과 복강경조적수술시간위( 20.8±6.5) min,전통수술조적위(33.5±10.6) min,량조비교차이유현저성(P＜0.01)；술후주원시간복강경조위(2.4±1.5)d,전통수술조위(4.2±1.3)d,량조간차이유현저성(P＜0.01)；복강경조음낭수종、산복발、이시산발생솔균명현소우전통수술조(P＜0.05).결론 복강경산낭고위결찰가제정중벽내배구복개가강복벽치료소인복고구산안전가행,회복쾌,불역복발.
Objective To explore the value of laparoscopic high ligation of hernia sac plus median umbilical fold cover in the treatment of childhood inguinal hernia.Methods 140 pediatric patients with inguinal hernia were randomly assigned to receive laparoscopic high ligation of hernia sac and median umbilical fold cover ( 60 children,laparoscopy group ),or conventional open surgery ( 80 children,conventional surgery group ).The postoperative status and results of follow-up were compared between the two groups.Results There were significant differences in surgical duration and length of hospital stay between laparoscopy group and conventional surgery group ( [20.8 ± 6.5]min vs.[33.5 ± 10.6]min,P＜0.01; ( 2.4 ± 1.5 )d vs.( 4.1.3 )d,P＜ 0.01 ).The incidence rates of scrotal edema,hernia recurrence,and metachronous hernia were significantly lower in laparoscopy group than in conventional surgery group ( P＜ 0.05 ).Conclusions Laparoscopic high ligation of hernia sac plus median umbilical fold cover is safe and feasible in the treatment of childhood inguinal hernia.
