目的观察乙型肝炎肝硬化患者左右半肝体积及肿瘤坏死因子(TNF)-α和白细胞介素(IL)-6的表达,探讨肝硬化患者左右半肝硬化程度.方法对45例乙型肝炎肝硬化患者和15例正常对照组的肝体积及TNF-α和IL-6进行分析.结果 45例肝硬化患者中右半肝萎缩较左半肝萎缩明显增多(P＜0.0l).TNF-α在病例组右半肝的表达明显高于病例组左半肝的表达,差异有统计学意义(x2=30.42,P＜0.0l),IL-6在病例组右半肝表达明显高于病例组左半肝表达,差异有统计学意义(x2=16.20,P＜0.01).结论肝硬化时左右半肝纤维化程度存在明显差异,右半肝较左半肝纤维化程度显著.

Objective To investigate the morphous of left and right half-liver cirrhosis and the expression of tumor necrosis factor (TNF)-α and interleukin (IL)-6.Methods The liver volume,TNF-α and IL-6 were analyzed in 45 specimens of patients with liver cirrhosis and 15 specimens of controls.Results In the 45 cases of liver cirrhosis,the right lobe of liver had a more significant sign of atrophy than the left lobe of liver.The expression of TNF-α in the patients group was more obvious than that in the control group (x2 =30.42,P ＜0.01 ).The expression of IL-6 in the patients group was significantly higher than in the control group (x2 =16.20,P ＜ 0.01 ).Conclusion The difference in the degree of left and right half-liver hepatic fibrosis in hepatic cirrhosis was significant.There is a close relationship between TNF-α,IL-6 and left and right half-liver fibrosis in hepatic cirrhosis.This research provides some new theoretical basis for the location and the size of the surgical excision of hepatocarcinoma with cirrhosis.
