目的 总结吲哚菁绿术中荧光造影辅助下脊髓血管畸形的手术疗效.方法 自2009年8月至2011年5月,共收治脊髓血管畸形24例,在吲哚菁绿术中荧光造影辅助下行手术切除畸形血管团或夹闭畸形血管瘘口,对其疗效进行随访.结果 除1例外,术中荧光造影确认瘘口完全夹闭或髓内畸形血管团完全切除.脊髓正常血供和静脉回流得以满意保留.21例获得随访,术后3例失访.其中痊愈6例,改善10例,稳定2例,加重3例.结论 吲哚菁绿术中荧光造影可以有效地提高脊髓血管畸形的手术疗效.

목적 총결신타정록술중형광조영보조하척수혈관기형적수술료효.방법 자2009년8월지2011년5월,공수치척수혈관기형24례,재신타정록술중형광조영보조하행수술절제기형혈관단혹협폐기형혈관루구,대기료효진행수방.결과 제1예외,술중형광조영학인루구완전협폐혹수내기형혈관단완전절제.척수정상혈공화정맥회류득이만의보류.21례획득수방,술후3례실방.기중전유6례,개선10례,은정2례,가중3례.결론 신타정록술중형광조영가이유효지제고척수혈관기형적수술료효.
Objective To summarize the surgical treatment of spinal vascular malformation assisted with intraoperative indocyanine green (ICG) videoangiography.Methods ICG videoangtography was performed during 24 surgical interventions of spinal vascular malformations from August 2009 to May 2011.The vascular malformations were removed or the fistulae were occluded with the assistance of ICG videoangiography.The completeness of fistulae clipping or nidi extirpation and patients neurological status were evaluated. Results Intraoperative ICG videoangiography confirmed the definite clipping of the fistulous points or complete removal of intramedullary AVMs in all but one case.Satisfactory preservation of spinal cord blood supply and venous return were appreciated.Excluding three patients lost to follow -up,21patients were followed clinically with a mean of 7.5 months.The neurological deficits disappeared in 6,improved significantly in 10,remained stable in 2,and aggravated in 3 cases.Conclusion Intraoperative ICG videoangiography can efficiently improve the therapeutic effect in the surgical treatment of vascular malformation of spinal cord.
