目的观察即刻肝素盐水灌注法对血液透析过程中动-静脉内瘘血栓形成的治疗作用.方法对25例在血液透析过程中发生内瘘闭塞的患者,在发现内瘘闭塞时保留穿刺针,立即采用普通肝素50 mg+生理盐水20 ml稀释,从内瘘的静脉端缓慢注入并观察内瘘的恢复情况,同时监测凝血酶原时间、活化部分凝血活酶时间水平的变化.结果 25例闭塞的动-静脉内瘘患者有18例抢救成功,内瘘处可重新触及震颤、闻及血管杂音.内瘘再通率达到72%,无出血现象.结论即刻肝素盐水灌注对透析过程中形成的内瘘血栓具有确切的溶栓作用,其操作方法简单,溶栓成功率较高,具有一定的安全性.

Objective To observe the therapeutical effect of instantly heparin normal sodium irrigation method on thromb which was formed in internal arteriovenous during dialysis process. Meathods 25 patients whose internal arteriovenous fistula were obstructived during hemodialysis, when the arteriovenous fistula obstruction was discovered, retained the puncture needle, used ordinary heparin 50 mg diluted with physiological saline 20ml immediately, poured into slowly from the fistula's vein end, observed the fistula restoration situation, and monitored PT, APTT changes. Results Among 25 examples obstructived fistula there were 18 examples were successfully rescued, in the fistula place may touch chatter, heared vascular murmur, the recanalization rate achieved 72%, non-hemorrhage phenomenon. Conclusion Instantly the heparin normal sodium irrigation in fistula thrombus which formed during the dialysis process that had accurately thrombolysis function, its operating procedure was simple, dissolved the thromb success ratio to be high, had certain security.
