目的探讨损伤控制外科理念在老年急性胆管炎患者治疗中的应用.方法对2000年1月至2010年6月收治的46例老年急性胆管炎患者的早期复苏、手术时机及手术方法进行回顾性分析.结果术后死亡2例,胆道残余结石5例,术后出现并发症12例,其中肺部感染8例,切口感染4例.结论老年急性胆管炎患者合并症多,诊断一旦明确,在损伤控制外科理论的指导下进行复苏,治疗基础病,抗感染,纠正休克、水电解质及酸碱平衡紊乱和凝血功能异常,尽早手术治疗.手术尽量简单有效,达到减压引流的目的.

Objective To investigate the application of the theory of the damage control surgery in acute cholangitis of elderly. Methods Regarding the 46 cases of acute cholangitis of elderly from January 2000 to June 2010, analyze the early recovery, surgery timing and method of surgery. Results Two cases of death after surgery, 5 cases of residual bile duct stones, 12 cases of complication after surgery, including 8 cases of lung infection and 4 cases of surgical incision infection. Conclusions High likelihood of complication for acute cholangitis of elderly patients, diagnosis once confirmed, under the theory of the damage control surgery, begin recovery, curing background diseases, prevention of infection, recovery from shock, imbalance of water-electrolyte, imbalance of acid-base, and abnormal blood coagulation, should execute surgery early. Surgery should be simple and effective, achieving reduction of pressure and induction of flow.
