目的 重复阻断人一侧上臂血液供应,监测循环血中腺苷含量变化,探讨局部缺血预适应对全身的保护效应.方法 选取5例健康成年自愿者制备人体重复上臂压迫模型,用袖带压迫肱动脉,加压至200 mm Hg(1 mm Hg=0.133 kPa),持续5 min、间隔5 min,重复5次,采用高效液相色谱(HPLC)的方法,观察压迫前后血浆中腺苷(ADO)含量的变化,并记录受试者的主观感受.结果 HPLC结果 显示,重复上臂压迫前后ADO含量呈现规律性变化,在5次压迫结束后ADO含量升高,在压迫后15、30 min降低,在60 min达到最高值(1069.622μg/L),达4.44±0.19倍于压迫前(P<0.01).在压迫后24 h血浆中ADO含量依然保持高水平.循环血腺苷含量升高的同时,受试者缺血侧上臂的不适感却明显降低(P<0.01).结论 上臂血供重复阻断中,血浆腺苷含量显著提升,缺血肢对缺血的耐受显著增强,提示重复阻断一侧上臂血供可激发全身性远程异位低氧预适应的保护物质.

Objective To establish a simple model of brief upper arm compression in humans, detecting the changes of concentration of serum adenosine (ADO) ,to study the general protection of local ischemic preconditioning. Methods The study was performed on 5 healthy adult volunteers. Isehemie state was made on the upper ann by inflating a blood pressure cuff to 200 mmHg for five 5 min periods, separated by 5 min of reperfusion. The feelings of the volunteers were documented during the compressions. The venous blood at different time points was obtained. HPLC was used to measure the concentration of ADO in the circulating blood. Results ADO was regularly increased ,and reached the peak value of 1069. 622 μg/L at p-60 rain (P < 0.01) and sustained the high level in p-24h group. Meanwhile the uncomfortable feeling of volunteers decreased. The results of the study show that remote ischemic preconditioning can increase the tolerance of the arm to ischemic injury. This effect may contribute to the protective effect of isehemic preconditioning against ischemia-perfusion injury.
