目的 探讨高压氧(HBO)治疗对急性脑出血患者高敏C反应蛋白(hs-CRP)及血糖(BG)的影响.方法 脑叶血患者112例(其中HBO治疗组55例,常规治疗组57例)及正常对照组50例,分别观察治疗前及治疗后不同时段血清hs-CRP、BG含量的变化并进行治疗前和治疗后第15、30天进行神经功能缺损评分(NIHSS)及日常生活活动能力评分(ADL).结果 常规治疗组[(15.68±4.96)mg/L,(9.85±4.32)mmol/L]和HBO组[(15.72±5.11)mg/L,(9.81±4.45)mmol/L]入院当日的血清hs-CRP、BG含量均高于正常对照组[(3.76±1.25)mg/L,(4.28±1.12)mmol/L](P<0.01),但2组间差异无统计学意义;治疗后2组hs-CRP、BG含量均明显下降,HBO组[(4.26±1.72)mg/L,(4.37±1.08)mmol/L]下降较常规治疗组[(5.39 ±2.14)mg/L,(4.79±1.16)mmoL/L]更为显著,差异有统计学意义(P<0.05),HBO组治疗2个疗程后hs-CRP、BG含量基本恢复正常,与正常对照组相比差异无统计学意义(P>0.05).治疗后15、30 d,HBO组NIHSS、BI(15 d:9.75±2.12,55.40±11.26;30 d:6.23±1.85,75.03±12.16)较常规治疗组(15 d:11.24±2.17,48.23±10.52;30 d:8.04±1.96,65.18±11.52)均有显著改善(P<0.01).结论 HBO治疗能有效降低脑出血患者血清hs-CRP及BG含量,改善患者神经功能缺损,提高日常生活活动能力.

Objective To study the effects of hyperbaric oxygen (HBO) therapy on the contents of serum high sensitivity C-reactive protein ( hs-CRP ) and blood glucose ( BG ) in patients with intracerebral hemorrhage. Methods One hundred and twelve were selected as subjects, and divided into routine treatment group (57 cases) and HBO treatment group (55 cases), and 50 healthy subjects were chosen as the control.Then, observations were made on the contents of serum hs-CRP and BG in subjects before and after therapy.The therapeutic efficacy was evaluated by scores of neurologic impairment (NIHSS) and activities of daily life (ADL) before treatment and on the 15th and 30th days after treatment. Results The contents of serum hs-CRP and BG in routine and HBO groups were all higher than those of the control group on the day of admission ( P <0. 01 ), but there was no statistical difference between the 2 groups. Contonts of serum bs-CRP and BG in both groups decreased significantly after therapy, with the reduction of data being sharper in HBO group and statistical difference could be seen between both groups (P <0. 05 ). For the HBO group, contents of serum hs-CRP and BG almost returned to normal after 2 sessions of HBO therapy, and no statistical significance was noted when compared with those of the control group (P < 0. 05 ). There was significant improvement in NIHSS and ADL in the HBO group, when compared with those of the routine group on the 15th and 30th days after treatment ( P < 0. 01 ). Conclusions HBO treatment can effectively reduce the contents of serum hs-CRP and BG in patients with intracerebral hemorrhage, improve neurologic impairment and activities of patients' daily life.
