目的 探讨乙型肝炎病毒相关性肾炎(HBV-GN)足细胞顶端膜蛋白podoealyxin的改变及拉米夫定干预后的变化.方法 将67例HBV-GN患者作为观察组;60例健康查体者为对照组,观察尿中足细胞计数和podocalyxin(PCX)的改变及拉米夫定治疗后的变化.结果 67例不同病理类型患者尿中足细胞计数和PCX的含量均较对照组为高;与尿蛋白定量呈正相关;拉米夫定治疗后尿中足细胞计数和PCX的含量均较治疗前明显下降.结论 HBV-GN患者尿足细胞计数及PCX的变化与肾脏组织的损伤程度相关;通过拉米夫定的抗病毒治疗有利于损伤足细胞的修复;尿足细胞计数及尿PCX是反映肾组织病变的灵敏指标.

Objective To observe the changes of the glomerular pedocyte apical membrane protein podocalyxin in patients with HBV-associated glomerulonephritis and the effect of intervention of Lamivudine.Methods To analyze the podocyte count and the expression of podocalyxin in the urine and changes after treat-ment of Lanivudine. The observation group consisted of 67 patients with HBV-associated glomerulonephritis and 60 persons who did health examination in the same time were used as control group. Results The podocyte count and the podocalyxin content in the observation group were higher than those in the control group and a positive correlation with urinary protein quantity; The podocyte count and the podocalyxin content descended ob-viously after treatment with Lamivudine. Conclusion There is a correlation between the podocyte count and the podocalyxin content of patients with HBV-associated glomerulonephritis and the extent of kidney damage; The antivirus therapy of Lamivudine benefits the repair of injured podocytes; The podocyte count and the podocalyxin content are sensitive indicators to the changes of nephropathy.
