目的 观察成人隐匿性自身免疫性糖尿病(LADA)患者胰岛β细胞功能及血清白介素6(IL-6)和肿瘤坏死因子α(TNF-α)水平的变化,探讨细胞因子在LADA发病中的作用.方法 分别测定LADA患者32例、2型糖尿病(T2DM)患者30例、健康人(NC)30名的血清胰岛索(Ins)、C-肽(C-P)和IL-6、TNF-α的水平.结果 LADA组空腹、餐后Ins和C-P均明显低于T2DM组(P<0.01或P<0.05);NC组、T2DM组、LADA组的IL-6和TNF-α水平依次增高(P<0.05或P<0.01).结论 LADA患者的胰岛β细胞功能损伤比T2DM患者严重.IL-6、TNF-α可能参与了LADA的自身免疫过程及T2DM的免疫炎症过程.

Objective To observe the changes in islet cell function and serum IL-6, TNF-α levels of latent autoimmune diabetes in adults (LADA) and the effect of the cytokines on the pathogenesis of LADA. Methods 32 LADA patients,30 type 2 diabetic patients and 30 healthy subjects underwent the detections of serum insulin, C-peptide, IL-6,and TNF-α. ResultsSerum level of fasting and postprandial insulin and C-peptide were lower in LADA than in type 2 diabetes (P<0.05 or P<0.01). The serum IL-6 and TNF-α levels were gradually higher one by one in three groups of healthy subjects,T2DM and LADA. Conclusions The β-cell function damage is more serious in LADA than in type 2 diabetes. IL-6, TNF-α may be involved in the LADA autoimmune process and in the immune inflammatory process of type 2 diabetes.
