目的 初步探讨代谢综合征(MS)与阻塞型睡眠呼吸暂停综合征(OsAs)的相关性.方法 测定并分析82例OSAS患者中43例MS患者(MS组)和39例非MS患者(非MS组)的睡眠呼吸监测指标、MS的组成成分等.结果 (1)MS组的腰围(wc)、体重指数、收缩压(SBP)、inTG、InFBG均高于非MS组(P<0.05或<0.01),高密度脂蛋白胆固醇(HDL-C)低于非MS组(P<0.05).(2)MS组的睡眠呼吸暂停低通气指数(AHI)高于非MS组,最低脉氧饱和度(LSpO2)低于非MS组[分别为(43.95±19.18)次/h比(10.58±3.52)次/h、0.67±0.15比0.82±0.07,P<0.01].(3)3个代谢异常组分时,AHI高于0~2个代谢异常组分、LSpO2低于0~2个代谢异常组分(P<0.01);代谢异常组分数进一步增加到4~5个时,AHI进一步增高、LSpO2进一步降低(P<0.05).(4)AHI与InFBG、WC、SBP呈正相关(P<0.01),与HDL-C呈负相关(P<0.01).(5)LSpO2与InFBG、WC、SBP呈负相关(P<0.01),与HDL-C呈正相关(P<0.05).结论 MS和OSAS关系密切.

목적 초보탐토대사종합정(MS)여조새형수면호흡잠정종합정(OsAs)적상관성.방법 측정병분석82례OSAS환자중43례MS환자(MS조)화39례비MS환자(비MS조)적수면호흡감측지표、MS적조성성분등.결과 (1)MS조적요위(wc)、체중지수、수축압(SBP)、inTG、InFBG균고우비MS조(P<0.05혹<0.01),고밀도지단백담고순(HDL-C)저우비MS조(P<0.05).(2)MS조적수면호흡잠정저통기지수(AHI)고우비MS조,최저맥양포화도(LSpO2)저우비MS조[분별위(43.95±19.18)차/h비(10.58±3.52)차/h、0.67±0.15비0.82±0.07,P<0.01].(3)3개대사이상조분시,AHI고우0~2개대사이상조분、LSpO2저우0~2개대사이상조분(P<0.01);대사이상조분수진일보증가도4~5개시,AHI진일보증고、LSpO2진일보강저(P<0.05).(4)AHI여InFBG、WC、SBP정정상관(P<0.01),여HDL-C정부상관(P<0.01).(5)LSpO2여InFBG、WC、SBP정부상관(P<0.01),여HDL-C정정상관(P<0.05).결론 MS화OSAS관계밀절.
Objective To explore the correlation between metabolism syndrome (MS) and obstructive sleep apnea syndrome (OSAS). Methods Forty-three MS subjects (MS group) and 39 non-MS subjects (non-MS group) were involved in this study. Polysomnography parameters such as apnea hypopnea index (AHI),the lowest pulse oxygen saturation (LSpO2),MS-associated parameters such as lipid,fasting blood ghcose (FBG), blood pressure, and some body indexes such as body mass index (BMI)and waist circumference(WC)wcre measured in all cases. Results (1) WC, BMI, SBP, InTG and InFBG were higher in MS group than those in non-MS group (P < 0.05 or < 0.01), HDL-C was lower in MS group than that in non-MS group (P < 0.05). (2)AHI in MS group was significantly higher than that in non-MS group,while LSpO2 in MS group was significantly lower than that in non-MS group [(43.95±19.18) times/h vs (10.58±3.52) times/h, 0.67±0.15 vs 0.82±0.07, respectively, P < 0.01)]. (3)AIR increased and LSpO2 decreased obviously in 3 kinds of abnormal metabolic ingredients group than that in 0-2 kinds of abnormal metabolic ingredients group (P< 0.01). Moreover, AHI further increased and LSpO2 decreased obviously in 4-5 kinds of abnormal metabolic ingredients group than that in 3 kinds of abnormal metabolic ingredients group (P <0.05).(4) AHI was positively correlated with InFBG, WC, SBP and negatively correlated with HDL-C (P < 0.01). (5) LSpO2 was negatively correlated with InFBG, WC, SBP (P < 0.01) and positively correlated with HDL-C(P< 0.05). Conclusion There is close relationship between MS and OSAS.
