目的 观察慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)不同时期诱导痰中白介素8(IL-8)、可溶性细胞间黏附分子1(sICAM-1)水平的变化情况,探讨两者对COPD病情评估所起的作用.方法 20名健康人为对照组,40例COPD患者,采用ELISA法检测对照组、急性加重期及治疗稳定后2周、4周、8周、12周诱导痰中IL-8、sICAM-1的水平,并常规检测肺功能.结果 COPD组治疗前、后与对照组间的诱导痰液IL-8、sICAM-1水平相比,差异有统计学意义(P=0.007,P=0.041; P=0.016,P=0.032),治疗稳定后不同时间点间IL-8、sICAM-1水平的差异均无统计学意义(P=0.078,P=0.063).结论 COPD急性加重期IL-8、sICAM-1水平明显升高,可能参与了COPD气道炎症的过程,治疗稳定后2周及更长时间内炎症介质水平趋于稳定.

Objective To observe the induced sputum interleukin-8 (IL-8), soluble intercellular adhesion molecule-1 (sICAM-1) levels in different periods of chronic obstructive pulmonary disease (COPD) ,and investigate their role in evaluating the illness of COPD. Methods 20 healthy people as control group and 40 cases of COPD patients in acute exacerbation and 2 weeks, 4 weeks, 8 weeks, 12 weeks after treatment were detected by ELISA of their induced sputum IL-8, sICAM-1 levels, and routinely lung function were tested. Results IL-8, sICAM-1 levels obtained from induced sputum of control group compared with COPD group's before or after treatment, the differences were statistically significant ( P = 0. 007, P = 0. 041, P = 0. 016, P = 0. 032), IL-8 and sICAM-1 levels among COPD groups detected at different time points after treatment have no significant difference( P =0. 078, P =0. 063). Conclusions IL-8 and sICAM-1 level in acute exacerbation of COPD increase obviously, they may be involved in the process of airway inflammation in COPD, IL-8 and sICAM-1 levels tend towards stability from 2 weeks or even longer time after treatment.
