目的探讨Orem自护模式在施行人工全髋关节置换术的老年患者中的应用效果.方法将90例施行单侧人工全髋关节置换术的老年患者随机分为干预组和对照组各45例.干预组采用Orem自护模式,对照组采用常规护理方法,分别于入院时及术后7 d评价2组的自护意识、护理知识掌握情况、参与护理的主动性及进行HAMA量表评分,并观察2组病人的术后下床活动时间及出院时间.结果干预组患者的自护意识、护理知识掌握情况、参与护理主动性、HAMA量表评分均较对照组佳,且干预组患者的术后下床活动时间及出院时间较对照组短,经统计学分析,差异有统计学意义.结论将Orem自护模式应用于施行人工全髋关节置换术的老年患者,能缓解患者的焦虑情绪、提高自理能力,促使病人早日康复出院、提高生活质量.

Objective To explore the effect of Orem serf -care mode in senile patients undergoing artifi-cial total hip replacement. Methods 90 patients who would be undergoing unilateral artificial total hip replace-ment were divided into two groups randomly: 45 patients were in intervention group, treated with Orem serf -care mode;45 patients were in control group, treated with regular nursing. On the admission and 7th after operation, The patients in two groups were tested in their serf-preserve sense, nursing knowledge and initiative of participa-tion in nursing care, evaluated with the HAMA scales. The time of patients'movement and discharge after opera-tion were also observed. Results Compared with those in control group, the serf -preserve sense, nursing knowledge, initiative of participation in nursing care and HAMA score were improved significantly in intervention group. And the time of patients'movement and discharge after operation were also decreased. Conclusions Apply-ing the Orem self -care mode in senile patients undergoing artificial total hip replacement could relieve anxious e-motion, improve self-care ability, help patients recover earlier and improve life quality.
