目的 分析不同病程糖尿病患者血清D-3-羟丁酸水平与尿酮体含量的关系,探讨利于早期诊断及预防糖尿病酮症酸中毒的科学方法.方法 选择临床已确诊的不同病情糖尿病患者53例.其中无酮症酸中毒患者14例,轻度酮症酸中毒患者13例,中度酮症酸中毒患者17例,重度昏迷酮症酸中毒患者9例.分别检测其血清D-3-羟丁酸水平与尿酮体含量(半定量).结果 30.7%轻度酮症酸中毒患者的尿酮体结果仍为阴性(-),且11.2%的重度昏迷酮症酸中毒患者的尿酮体含量也只有(+),显然在诊断轻度酮症酸中毒时,血清D-3-羟丁酸的敏感性要远远高于尿酮体(P<0.05);而二者在诊断中、重度DKA的阳性率方面没有差别(P>0.05),但敏感性仍有一定差异.结论 D-3-羟丁酸在诊断糖尿病发生酮症酸中毒的敏感性要明显高于尿酮体,且其含量与病程的相关性也优于尿酮体,因此检测血清D-3-羟丁酸对于早期诊断及预防糖尿病酮症酸中毒有重大意义.

목적 분석불동병정당뇨병환자혈청D-3-간정산수평여뇨동체함량적관계,탐토리우조기진단급예방당뇨병동증산중독적과학방법.방법 선택림상이학진적불동병정당뇨병환자53례.기중무동증산중독환자14례,경도동증산중독환자13례,중도동증산중독환자17례,중도혼미동증산중독환자9례.분별검측기혈청D-3-간정산수평여뇨동체함량(반정량).결과 30.7%경도동증산중독환자적뇨동체결과잉위음성(-),차11.2%적중도혼미동증산중독환자적뇨동체함량야지유(+),현연재진단경도동증산중독시,혈청D-3-간정산적민감성요원원고우뇨동체(P<0.05);이이자재진단중、중도DKA적양성솔방면몰유차별(P>0.05),단민감성잉유일정차이.결론 D-3-간정산재진단당뇨병발생동증산중독적민감성요명현고우뇨동체,차기함량여병정적상관성야우우뇨동체,인차검측혈청D-3-간정산대우조기진단급예방당뇨병동증산중독유중대의의.
Objective Analyzes the different course diabetic blood serum D-3-HB level and the urine KET content relations,the discussion favors the early diagnosis and the prevention DKA scientific method Methods Chooses clinical already the different condition diabetic 53 examples which diagnoses,does not haye the alkone sickness acidosis patient 14 examples,mild alkone sickness acidosis patient 13examples,moderate alkone sickness acidosis patient 17 examples,specific weight stupor alkone sickness acidosis patient 9 examples.Distinction examination his/her blood serum D-3-HB level and urine KET content(semi-quantitative).Results 30.7%mild alkone sickness acidosis patient's urine KET result still for negative(-),and 11.2%specific weight stupor alkone sickness acidosis patient's urine KET content also onlv then(+),obviously in diagnosis mild alkone sickness acidosis time,blood serum D-3-HB's sensitivity must he higher than the urine KET by far,But the two in the diagnosis,the specific weight DKA masculine gender rate aspect do not have the difference,but the sensitivity still had certain difference Conclusions The D-3-HB has the alkone sickness acidosis sensitivity at the diagnosis diabetes to he higher than the urine KET obviously,and its content and the course relevance also surpasses the urine ketone body.Therefore the examination blood serum D-3-HB has the great significance regarding the early diagnosis and the prevention diabetes alkone sickness acidosis.
