目的研究顺铂静脉与腹腔内给药在体内不同组织的聚积性。方法 50例可切除进展胃癌的患者随机分成两组，腹腔给药组26例，静脉给药组24例。顺铂60mg/m2分别于术前腹腔及静脉给药，给药后160～170min，分别采取腹腔液、胃左静脉血、外周血；270～280min手术中取肿瘤组织、癌旁正常组织。另外8例也分两组分别作顺铂腹腔内给药与静脉给药的药代动力学研究；各组织的总铂浓度用石墨炉原子分光光谱分析法测定。结果给药后160～170min，腹腔给药组腹腔液、胃左静脉的总铂浓度明显高于静脉给药组(P＜0.001)，分别高出静脉给药组的24.8、2.35倍；给药后270～280min，腹腔内给药组肿瘤组织内的浓度比静脉给药组高，且有显著意义(P＜0.05)。结论顺铂腹腔内给药可作为进展期胃癌有效的辅助治疗手段。
목적연구순박정맥여복강내급약재체내불동조직적취적성。방법 50례가절제진전위암적환자수궤분성량조，복강급약조26례，정맥급약조24례。순박60mg/m2분별우술전복강급정맥급약，급약후160～170min，분별채취복강액、위좌정맥혈、외주혈；270～280min수술중취종류조직、암방정상조직。령외8례야분량조분별작순박복강내급약여정맥급약적약대동역학연구；각조직적총박농도용석묵로원자분광광보분석법측정。결과급약후160～170min，복강급약조복강액、위좌정맥적총박농도명현고우정맥급약조(P＜0.001)，분별고출정맥급약조적24.8、2.35배；급약후270～280min，복강내급약조종류조직내적농도비정맥급약조고，차유현저의의(P＜0.05)。결론순박복강내급약가작위진전기위암유효적보조치료수단。
Objective　To examine the accumulation of cisplatin in different tissues by different administrative routes.Method　50 patients with advanced respectable gastric cancer were randomly divided into 2 groups.One guoup was treated by preopertive intraperitoneal cisplatin (ip group),which contained 26 cases.The other group was treated by preoperative intravenous cisplatin (iv group),which cntained 24 cases.60mg/m2 cisplatin was given to each patient intraperitoneally or intravenously.Peritoneal fluid,blood from gastric sinistral vein and peripheral blood were taken on the 160th to 170th minutes after the drug was administrated.Cancer tissues and peritumor normal tissues were collected during operation on the 270th to 280th minutes after the drug given.The concentrations of cisplatin in different samples were measured by the flameless type of atomic absorption spectrometry.Result　The cisplatin concentrations of peritoneal fluid,gastric sinistral vein blood and peripheral blood in the ip group were respectively 24.8,2.35 and 1.5 times higher than those in the iv group (P<0.01).The cisplatin of the cancer tissue in the ip group was higher than that in the iv group(P<0.05).Conclusion　The intraperitoneal cisplatin chemotherapy could be a useful accessory treatment for the advanced gastric cancer.
