目的 探讨雌激素对绝经后妇女脂代谢的影响。方法25例绝经后妇女,其中15例为口服结合雌激素组(研究组),10例为不用药组(对照组),观察血浆总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白-胆固醇(HDL-C)、低密度脂蛋白-胆固醇(LDL-C)和α-脂蛋白(a-Lp)及TC/HDL-C、LDL-C/HDL-C比值的变化;观察研究组血清FSH、LH、E2和E1的变化。 结果激素替代疗法(HRT)后,HDL-C随用药时间的延长上升,TC/HDL-C及LDL-C/HDL-C比值明显降低,α-LP下降,尤其在第6个月后。研究组血清FSH和LH 3个月后即显著下降,而E2和E1明显升高(P<0.01)。 结论绝经后妇女使用雌激素 0.625mg/d,合并安宫黄体酮4mg/d对升高HDL-C,降低TC/HDL-C、LDL-C/HDL-C比值与α-LP的下降起着有益的作用,从而对绝经后妇女冠心病的发生起到预防作用。

목적 탐토자격소대절경후부녀지대사적영향。방법25례절경후부녀,기중15례위구복결합자격소조(연구조),10례위불용약조(대조조),관찰혈장총담고순(TC)、감유삼지(TG)、고밀도지단백-담고순(HDL-C)、저밀도지단백-담고순(LDL-C)화α-지단백(a-Lp)급TC/HDL-C、LDL-C/HDL-C비치적변화;관찰연구조혈청FSH、LH、E2화E1적변화。 결과격소체대요법(HRT)후,HDL-C수용약시간적연장상승,TC/HDL-C급LDL-C/HDL-C비치명현강저,α-LP하강,우기재제6개월후。연구조혈청FSH화LH 3개월후즉현저하강,이E2화E1명현승고(P<0.01)。 결론절경후부녀사용자격소 0.625mg/d,합병안궁황체동4mg/d대승고HDL-C,강저TC/HDL-C、LDL-C/HDL-C비치여α-LP적하강기착유익적작용,종이대절경후부녀관심병적발생기도예방작용。
Objective To investigate the effect of hormone replacement therapy on plasma lipidand lipoprotein metabolism. . Methods 25 healthy postmenopausal women were studied. 15 womenwere given conjugated estrogen (CE) 0. 625mg/d and medroxyprogesterone acetate (MPA) 4mg/d for12 months (HRT group) while 10 cases didn't receive any hormonal medication (control group). Theplasma levels of total cholesterol (TC), triglyceride(TG), high- density lipoprotein cholesterol(HDL-C), low-density lipoprotein- cholesterol(LDL-C) and α- lipoprotein (α- Lp) were evaluated at 0,3,6,and 12month, respectively, and plasma FSH, LH, E2, and E1 levels were measured at the samepoint in both groups. Results In HRT group TC and TG levels were slightly increased, but no sig-nificant difference was detected. HDL- C was significantly increased in HRT group while TC/HDL- Cand LDL-C/HDL-C ratios were significantly dereased. FSH and LH levels were lowered, howerverEa and E1 levels were elevated at 3 month ( P < 0.01 ). Conclusion Hormone replacement therapyhas a potential protective effect on decreasing the morbidity of coronary heart disease in postmenopausalwomen by increasing plasma HDL-C level and decreasing TC/H L-C and LDL-C/HDL-C ratios
