目的 探讨Grave眼病下睑退缩并睑内翻倒睫的最佳手术方式,并避免术后复发.方法 对确诊Grave眼病下睑退缩并内反倒睫的患者手术治疗.从下睑缘下1.5 mm处作皮肤切口,沿下睑板下缘切断下睑缩肌与睑板肌复合体,充分游离后,在下睑板下缘与复合体间填充一条同种异体巩膜片,然后缝合皮肤创口.结果 治疗初诊患者16例,外院复发患者5例,其中男性4例,女性17例,年龄18～60岁,单侧5例,双侧16例.手术后经3～46个月观察,下睑位置恢复正常,未见复发病例.结论 此手术不仅松解了处于兴奋状态的睑板肌,更主要的是阻断了交感神经对睑板肌的作用.不仅近期疗效好,也预防了疾病复发.是治疗Grave眼病下睑退缩并睑内翻倒睫的一种较好手术方法.

목적 탐토Grave안병하검퇴축병검내번도첩적최가수술방식,병피면술후복발.방법 대학진Grave안병하검퇴축병내반도첩적환자수술치료.종하검연하1.5 mm처작피부절구,연하검판하연절단하검축기여검판기복합체,충분유리후,재하검판하연여복합체간전충일조동충이체공막편,연후봉합피부창구.결과 치료초진환자16례,외원복발환자5례,기중남성4례,녀성17례,년령18～60세,단측5례,쌍측16례.수술후경3～46개월관찰,하검위치회복정상,미견복발병례.결론 차수술불부송해료처우흥강상태적검판기,경주요적시조단료교감신경대검판기적작용.불부근기료효호,야예방료질병복발.시치료Grave안병하검퇴축병검내번도첩적일충교호수술방법.
Objective To find out the best operation for the retraction and entropion of the lowerlids of Grave's ophthalmopathy,and avoid recurrence after operation. Methods The patient was diagnosed with Grave's ophthamolpathy according to the symptom of the retraction of eyelids,exophthalmos and CT showed the hypertrophy of extraocular muscle,the increase of density of the peaked orbit and the widen of optic nerve.The skin was cut from 1.5 mm below the lowerlid,the complex of the lowerlid retraction and tarsal muscle was cut off along with inferior border of lower tarsi,the tissue was dissociated fully,and a strip of allogenotic sclera was placed with inferior border of lower tarsi and the complex,then the skin cut was sewed up. Results There were 21 patients,16 cases were the first visit and 5 cases were the second or third visit.It was found in 4 male and 17 female patients aging from 18 to 60 years old.Five cases were single eyelid,and 16 cases were double eyelids.None of case recurred through observing from 3 to 46 months.After operation the position of lowerlid resumed normal. Conclusions The operation not only relaxes the excited tarsal muscle but also interrupt the sympathetic nerve for tarsal muscle.The method is extremely effective in the near future and can prevent the recurrence of disease.So it is a good treatment for retraction and entropion of the lowerlids of Grave's ophthamolpathy.
