目的 联合检测抗CCP抗体和AKA,评价血清学临床诊断意义.方法 对320例有症状性关节痛或关节炎的患者,ELISA法检测抗CCP IgG抗体,间接免疫荧光法检测AKA,ELISA法检测RFIgA、IgG、IgM.结果 抗CCP抗体、AKA、RF的阳性率分别为25.9%(83/320)、30.3%(97/320)、43.4%(139/320),30例正常对照中,仅2例RF阳性,1例CCP阳性,其余指标均为阴性.三种抗体同时阳性率23.8%(76/320),明显高于正常对照组(P<0.01).结论 抗CCP抗体和AKA并同时检测RF IgA、IgG、IgM,有助于诊断和早期诊断,并可作为临床治疗动态观察指标.

Objective To assess the significance of combined detection of anti-cyclic citrullinated peptide antibody and anti-keratin antibody in the diagnosis of rheumatoid arthritis. Methods A total of there hundred and twenty patients with established RA was enrolled. Serum anti-cyclic citrullinated peptide(CCP) antibodies and IgA, IgGand IgM rheumatoid factor (RF)were measured by enzyme-linked immunosorbent assay (ELISA). Anti-keratin antibody(AKA) was detected by indirect immunofluorescence assay using rat esophagus as substrate. Results The prevalence of various autoantibodies was: IgM RF 82. 2% , anti-CCP antibodies 30. 3% ,AKA 25.9% ,and RF 43.4%. All but one healthy control sample were positive for AKA and two were positive for RF. 76 of the 320 patients(23.3% ) were positive for the three kinds of antibodies and the presence was significantly higher than the healthy control group( P <0.01 ). Conclusion The combined detection of anti-CCP and AKA, in addition to RF was useful in establishing early diagnosis of RA, and can be a useful parameter in the clinical therapy.
