目的 探讨血清免疫球蛋白IgM和C反应蛋白(CRP)联合测定在诊断新生儿早期感染性疾病中的诊断价值.方法 采刚免疫比浊法测定206例发生感染性疾病的新生儿(感染组)、146例非感染病症新生儿(未感染组)的血清IgM和CRP水平.结果 感染组CRP水平与非感染组比较,差异无显著性(P>0.05);感染组IgM水平与非感染组比较,差异有显著性(P<0.05).CRP对感染性疾病的诊断灵敏度Se为96.12%,特异度Sp为41.78%,准确度Ac为73.60%;IgM对感染性疾病的诊断灵敏度Se为88.29%,特异度SP为91.78%,准确度为Ac89.49%;两者联合测定,灵敏度SeCs为84.86%,特异度SpCs为95.21%.结论 血清IgM与CRP联合测定可作为诊断新生儿早期感染性疾病的指标.

Objective To explore the diagnostic value of combinatin detection of serum immunoglobin M (IgM) and C-reactive protein (CRP) for neonatal infectious diseases at the early stage.Methods Immunoturbidimetry was used to detect the serurn levels of IgM and CRP in 206 neonates with infectious diseases (infection group) and 146 with non-infectious diseases (non-infection group). Results There was no significant difference in CRP level between infection group and non-infection group (P>0.05);whereas IgM level differed significantly between the two groups (P<0.05). The sensitivity of CRP for infectious diseases was 96.12%, the specificity was 41.78%, and the accuracy was 73.60%; while the sensitivity ofIgM for infectious diseases was 88.29%, the specificity was 91.78% and the accuracy diseases was 89.49%. The sensitivity of combination detection was 84.86% and the specificity was 95.21%. Conclusions Combination detectinon of serum IgM and CRP can be used as an indicator for the early diagnosis of neonatal infectious diseases.diseases in newbrons.
