目的 评价电解可脱性铂金弹簧圈(GDC)栓塞颅内动脉瘤的效果。方法 在局麻或全麻及全身抗凝下,经导引导管将微导管头端送入动脉瘤腔,用GDC逐步将瘤腔填塞满意。术前、术中、术后造影对比,随访1~16个月。结果 22例共23个动脉瘤,栓塞21个,其中16个100%栓塞(76.2%),4个90%栓塞(19.0%),1个50%栓塞(4.8%);1个伴发动脉瘤未作处理;1个栓塞失败。死亡1例,余术后随访无SAH发生。4例动眼神经麻痹中3例完全恢复,1例稍有好转。结论 GDC栓塞治疗颅内动脉瘤安全可靠,效果良好,为颅内动脉瘤的治疗开拓了新途径。
目的 評價電解可脫性鉑金彈簧圈(GDC)栓塞顱內動脈瘤的效果。方法 在跼痳或全痳及全身抗凝下,經導引導管將微導管頭耑送入動脈瘤腔,用GDC逐步將瘤腔填塞滿意。術前、術中、術後造影對比,隨訪1~16箇月。結果 22例共23箇動脈瘤,栓塞21箇,其中16箇100%栓塞(76.2%),4箇90%栓塞(19.0%),1箇50%栓塞(4.8%);1箇伴髮動脈瘤未作處理;1箇栓塞失敗。死亡1例,餘術後隨訪無SAH髮生。4例動眼神經痳痺中3例完全恢複,1例稍有好轉。結論 GDC栓塞治療顱內動脈瘤安全可靠,效果良好,為顱內動脈瘤的治療開拓瞭新途徑。
목적 평개전해가탈성박금탄황권(GDC)전새로내동맥류적효과。방법 재국마혹전마급전신항응하,경도인도관장미도관두단송입동맥류강,용GDC축보장류강전새만의。술전、술중、술후조영대비,수방1~16개월。결과 22례공23개동맥류,전새21개,기중16개100%전새(76.2%),4개90%전새(19.0%),1개50%전새(4.8%);1개반발동맥류미작처리;1개전새실패。사망1례,여술후수방무SAH발생。4례동안신경마비중3례완전회복,1례초유호전。결론 GDC전새치료로내동맥류안전가고,효과량호,위로내동맥류적치료개탁료신도경。
Objective To assess the effect of intracranial aneurysm embolization with Guglielmi detachable coils(GDC). Methods Under local or general anesthesia and after systematic anticoagula- tion, a micro- catheter was navigated into the aneurysm through a guide catheter. Then, the aneurysm was embolized with GDCs which was introduced by the micro - catheter. Angiography was performed before, durng and after embolization. Patients were followed for 1 to 16 monthes. Results 21 of 23 aneurysms in 22 cases were embolized by GDCs. 100% occlusion was achieved in 16 aneurysms (76.2%), 90% in 4(19% ) and 50% in 1(4.8% ). One case failed to embolization. And a coincidental unruptured aneurysm was not embolized. One case died, and no rebleeding occured in this group after treatment. 3 of the 4 cases presented with oculomotor paralysis were completely recovered, another case was improved. Conclusion Embolization of intracranial aneurysms with GDCs is safe, effective and reliable. It has opened a new field for the treatment of intracranial aneurysm.
